摘  要:目的探讨多体素质子磁共振波谱(magnetic resonance spectroscopy,1H-MRS)技术测定听皮层不同区域代谢物分布的可行性。方法对10名健康志愿者右侧大脑听皮层的颞横回前外区(HGal)、颞横回后内区(HG-pm)、颞平面(PT)等区行二维多体素1 H-MRS检测,观察N-乙酰天门冬氨酸(NAA)、肌酸(Cr)、胆碱化合物(Cho)、乳酸(Lac)、谷氨酰胺和谷氨酸(glutamate/glutamine,Glx)、GABA等代谢物的波峰特点并进行半定量分析。完成检查后分别将以NAA/(Cho+Cr)、Lac/(Cho+Cr)、Glx/(Cho+Cr)、GABA/(Cho+Cr)等指标含量为灰阶的伪彩图与轴位T1WI进行融合,直观听皮层不同分区代谢物的分布情况。结果多体素1H-MRS研究发现NAA/(Cho+Cr)、Glx/(Cho+Cr)、GABA/(Cho+Cr)比值在听皮层颞横回前外区、后内区及颞平面各区含量不均匀。颞横回后内区Glx/GABA比值明显高于颞横回前外区、颞平面等,差异有显著统计学意义(t值分别为6.2672、6.6968,P<0.001)。通过视觉分析观察伪彩图与常规MRI的融合图像可见Glx、Lac的分布主要局限于颞横回后内区。结论二维多体素1 H-MRS能够显示听皮层不同分区代谢产物的代谢特征,为耳聋情况下听皮层代谢变化的研究提供重要的信息。Objective To study the feasibility of Multi voxel proton MR spectroscopy in measuring regional metabolic changes in normal auditory cortex. Methods 10 normal cases were examined by 2D muhi-voxel proton MR spectroscopy. N aceytl aspartate (NAA), creatine ( Cr), choline ( Cho ), lactate ( Lac ), glutamate/glutamine ( Glx ), GABA in three brain locations including antero-lateral aspect of first Heschl' s gyrus(HGal) ,postero-medial aspect of first Heschl' s gyrus (HG- pm) and planum temporale(PT) were calculated with half-quantitative analysis. The colour-coded imaging which related to metabolites and T1 imaging were fused. NAA/( Cho + Cr), Lac/( Cho + Cr), Glx/( Cho + Cr), GABA/( Cho + Cr) singal intensity in three brain locations were measured. The distribution of metabohtes in auditory cortex was analyzed. Results Heterogeneous distribution of NAA/( Cho + Cr), Glx/( Cho + Cr), GABA/( Cho + Cr), Lac/( Cho + Cr) and Glx/GABA in multi voxel PRESS sequence were found in the auditory cortex. The Glx/GABA ratio in postero-medial aspect of first Hes- chl ' s gyrus was significantly higher than that in the antero-lateral aspect of first Heschl' s gyrus and planum temporale ( P 〈 0.001 ). By studying the imaging fusion, Lac, Glx could be detected in postero-medial aspect of first Heschl' s gyrus. Conclusion 2D multivoxel proton MR spectroscopy could reflect the metabolic characteristics in each region of auditory cortex and provide valuable information in studying the cerebral biochemical changes in vivo in patients with various hearing loss.
