摘  要:目的探讨体表超声与体内超声的结合对提高胆管病变的诊断价值。方法回顾性分析体表超声检查未能明确诊断而行体内超声检查的患者65例,其中25例行内镜超声检查(endoscopic ul-trasonography,EUS),40例行胰胆管内超声检查(intraductal ultrasonography,IDUS)。结果体内超声的诊断准确率为93.85%,明显高于体表超声的诊断准确率52.31%。结论体表超声可作为胆管系统疾病的首选检查,当体表超声未能显示病变,但伴有胆管疾病症状及体征的患者可再行体内超声,弥补前者的不足,可提高胆管病变诊断的准确率。Objective To explore application of extracorporeal ultrasound to diagnosis of biliary duct diseases and the combination of intracorporeal ultrasound on the etiological diagnosis, thus evaluate the value of intracorporeal ul- trasound in improving the accuracy of the diagnosis for biliary duct diseases. Methods 65 patients, who were subject to intracorporeal ultrasound examination because of misdiagnosis by extracorporeal ultrasound examination, were ret- rospectively studied. 25 experienced EUS (endoscopic ultrasonography) examination and 40 were performed IDUS (in- traductal ultrasonography)examination. Results The coincidence of extracorporeal ultrasound method was 52.31%. The coincidence of intracorporeal ultrasound was 93.85%. Conclusions Extracorporeal ultralsound could serve as the principal screening examination method due to simple manipulation, non invasive, non-irradiation, low cost. intracor- poreal ultrasound examination could improve the accuracy of diagnosis and patients who have symptoms or physical signs of biliary diseases but without showing changes of extracorporeal ultrasound images may undergo intracorporeal ultrasound examination. The mixture of extracorporeal and intracorporeal ultrasound could improve the accuracy of di- agnosis for biliary diseases.
