摘  要:目的评价生物反馈电刺激结合盆底肌训练治疗法对初产妇产后盆底肌功能康复的效果,以及以盆底肌最大肌电压值和盆底肌持续收缩60s的平均肌电压值作为评价参数的可行性。方法选择2010年1月至2011年2月在福州市第二医院住院分娩60例初产妇,随机分成观察组(28例)和空白对照组(32例),其中观察组于产后42d开始进行生物反馈电刺激结合盆底肌训练治疗3个疗程,而空白对照组仅在产后42d进行一般性健康教育,未进行任何产后康复锻炼及治疗。然后分别测量两组产妇在产后42d和产后3个月的盆底肌最大肌电压和盆底肌持续收缩60s的平均肌电压。结果观察组经过治疗后盆底肌最大肌电压及盆底肌持续收缩60s的盆底肌平均肌电压,较空白对照组均有明显提高,差异有统计学意义(P<0.05)。结论产后早期进行生物反馈电刺激治疗结合盆底肌训练对产后盆底肌功能的康复有显著意义;以盆底肌最大肌电压值和盆底肌持续收缩60s的平均肌电压值作为评价参数,具有较好的可行性。Objective The primiparas in the postpartum period were treated by the multiple therapeutic methods of elec- tromyographic-feedbaek and pelvic floor muscle training. Base on the parameter of the maximum muscle voltage and the average muscle voltage in the physiology state of 60s continuous contraction of pelvic floor muscle, the therapeutic effect on the functional rehabilitation of pelvic floor muscles was evaluated. Methods 60 primiparas were randomly divided in- to a treated group (n = 28 ) and a control group (n = 32). Pelvic floor muscle of primiparas in the treated group was rem- edied by the multiple therapeutic methods of electromyographic-feedback and pelvic floor muscle training. While primipa- ras in the control group were not done any treatment except for knowledge dissemination about the sex health and the func- tion injury of pelvic floor muscle imposed by pregnancy and parturition. Both the maximum muscle voltage and the aver- age muscle voltage in the physiology state of 60s continuous contraction of 60 primiparas" pelvic floor muscle were meas- ured at 42 days postpartum and 90 days postpartum, respectively. Results Both the maximum muscle voltage and the average muscle voltage in the physiology state of 60s continuous contraction of pelvic floor muscle in the treated group were increased higher than that of the control group. There was significant difference between the treated group and the control group ( P 〈 0.05 ). Conclusions It is the multiple therapeutic methods of eleetromyographic-feedback and pelvic floor muscle training that effectively promote the functional rehabilitation of pelvic floor muscles in the early postpartum period of the primiparas. The parameter with the maximum muscle voltage and the average muscle voltage in the physiolo- gy state of 60s continuous contraction of pelvic floor muscle is objective.
