摘　　要：目的 比较股骨近端髓内钉- 螺旋刀片（PFNA）、动力髋螺钉（DHS）和人工股骨头置换术治疗高龄股骨转子间骨折的临床疗效,探讨内固定方法选择的策略.方法 分析自2003 年1月至2011 年1月收治分别使用PFNA、DHS 或人工股骨头置换术治疗的100例高龄股骨转子间骨折患者的临床资料,比较3 组患者手术时间、术中出血量、术后下地负重行走时间、术后并发症发生情况、骨性愈合时间及髋关节功能评分情况.结果 PFNA组在手术时间、术中出血量、术后下地负重行走时间、术后并发症发生情况、骨性愈合时间及髋关节功能评分情况等方面均优于DHS 和人工股骨头置换术组,差异均有统计学意义（均P＜0.05）.结论 PFNA 治疗高龄股骨转子间骨折疗效比较理想,为较好的内固定物.Objective To compare three surgical modalities for intertrochanteric fractures in elderly patients. Methods One hundred elderly patients with intertrochanteric fractures were treated with proximal femoral nail antirotation （PFNA） and dy- namic hip screw （DHS） or femoral replacement surgery from January 2003 to January 2011. The operation time, volume of blood loss, postoperative complications, bone healing, Harris score were retrospectively reviewed and compared among three modali- ties. Results The indicators of operation time, volume of blood loss, postoperative complications, bone healing and Harris score in PFNA group were better than those in other two groups （P〈0.05）. Conclusion PFNA provides satisfactory results and good internal fixations for intertrochanteric fractures in elderly patients.
