骨性关节炎的免疫水平与病变程度关系的初步研究

摘　　要：目的探讨骨性关节炎患者发病机制中的免疫因素及其与病变程度的关系。方法选取45例骨性关节炎患者作为实验组,按照轻、中、重病变程度不同分为三组,通过免疫法测定45例患者外周血CD4、CD8、IgG、IgM的含量及其中25例患者关节液中的CD4、CD8、IgG、IgM含量,同时选取10例健康者为对照组,测定其外周血的CD4、CD8、IgG、IgM的含量。结果实验组外周血中的CD4、CD8、IgG、IgM水平与对照组外周血比较,差异无统计学意义(P>0.05),而不同病变程度之间差异亦无统计学意义(P>0.05),但是关节液中CD4水平同外周血比较,差异有统计学意义(P<0.05),而CD8水平差异无统计学意义(P>0.05)。关节液中IgG、IgM水平与外周血中的水平及在不同病变程度之间比较,差异无统计学意义(P>0.05)。结论免疫因素可能参与骨性关节炎的发病机制,通过抑制免疫因素或许是未来治疗骨性关节炎的选择之一。Objective To investigate the relationship between the immunization level and the severity of osseous arthritis（OA）.Methods 45 cases of patients with knee osteoarthritis were selected as the experimental group and divided into three groups,according to the light,medium,serious extent of lesions.The peripheral blood percentage of CD4,CD8 and the levels of IgG,IgM of 45 patients with osseous arthritis were detected by immunifaction,among whom percentage of CD4,CD8 and the levels of IgG,IgM in joint fluid of 25 cases were detected.At the same time 10 normal person were selected as the control group and the peripheral blood percentage of CD4,CD8 and the levels of IgG,IgM were detected.Results Compared with the control group,the percentage of CD4,CD8 and the levels of IgG,IgM in peripheral blood of the experimental group had no significant differences（P 0.05） and the differences were also not significant among different extent of lesions groups（P 0.05）.But the level of CD4 in joint fluid was significantly different from the peripheral blood（P 0.05）,and the level of CD8 between the two places had no significant difference（P 0.05）.Levels of IgG and IgM in joint fluid were not significantly different from the peripheral blood（P 0.05）,and the differences were also not significant among different extent of lesions groups（P 0.05）.Conclusion The factor of immunity may participate in the pathogenesis of osseous arthritis,controlling the immune factors maybe the one of the selections in the treatment of osseous arthritis in the future.
