长期留置导管放置及使用并发症原因分析及处理

摘　　要：目的探讨长期深静脉留置导管在放置及使用中出现问题的原因和处理方法。方法对近年来我科长期导管放置失败、常见并发症及处理进行分析和总结。结果 47次置管手术中有2次置管失败,4例术后血流不畅;随访的36例患者中,1例因栓塞拔除,1例因卡夫脱出拔出,5例因原发病死亡,其余目前均正常使用。结论通过了解导管失败的原因,提高操作技术、正确处理导管使用常见的并发症,可以提高导管放置的成功率,延长导管的保留时间。Objective To study the causes of common questions and treatments about the long-term deep vein catheters during the placement and application. Methods Reviewed the long-termindwelling catheter placement cases in recent years, we analysed and summarized the causes of placement failure, common complications and treatments. Results Two cases of 47 （2/47） were failed to place long-termindwelling catheter. Four of which （4/47） happened poor blood flow after operation. The long-termindwelling catheter was removed for embolization in one case of 36 followed-up（1/36）. One（1/36） removed the catheter because the cuff fell off. 5 patients （5/ 36） died of protopathy. The rest （29/36） were using the catheters normally. Conclusion We summarized the causes of catheters failed. So we improved the operation technology and could correctly deal with common compli- cations, we increased the success rate of the catheter placement and prolonged the catheter life span.
