氯沙坦钾对高血压左室肥厚及神经内分泌激素的影响

摘　　要：目的　观察氯沙坦钾 (科素亚 )对高血压左室肥厚及神经内分泌激素的影响。方法　 2 6例高血压伴左室肥厚病人服用科素亚 5 0～ 1 0 0 mg/ d,共 8周 ,治疗前后分别测定左室重量指数 (L VMI)、血浆肾素活性 (PRA )、血管紧张素 (Ang )及醛固酮 (AL D)水平。结果　与治疗前相比 ,L VMI明显减小 [(1 2 0 .0 8± 1 5 .35 ) g/ m2 ∶ (1 4 7.2 1± 1 6 .72 ) g/ m2 ,P<0 .0 5 ) ],AL D水平明显下降 [(5 4.2± 2 5 .5 ) pg/ ml∶ (96 .8± 2 7.4) pg/ m l,P<0 .0 1 ) ],差异有显著性 ,而 PRA和 Ang 水平稍有增加 ,差异无显著性 (P>0 .0 5 )。结论　科素亚具有降低血压、调节神经内分泌激素水平和逆转左室肥厚的作用。Objective To study the effects of Cozaar (Iosartan potassium) on left ventricular hypertrophy (LVH) and neuroendocrinologic hormones in essential hypertensive patients. Methods 26 patients with essential hypertension accompanied with LVH were given Cozaar 50~100 mg once daily for 8 weeks. Left ventricular mass index (LVMI) was assessed by echocardiography, the levels of plasma renin activity(PRA), angiotensinⅡ(AngⅡ) and aldosterone (ALD) were measured by radioimmunoassay. Results After 8 weeks the LVMI in the essential hypertensive patients was significantly lower than that before [(120.08±15.36)vs(147 21±16 72) g/m\+2,\%P\%<0 05, respectively], and the level of ALD decreased [(54.2±25 5)vs(96.8±27 4) pg/ml, P <0 01, respectively], while the levels of PRA and AngⅡ slightly increased (\%P>0.05\%) and there were no significant difference. Conclusion Cozaar may lower blood pressure, regulate the levels of neuroendocrinologic hormones and reverse the LVH.
