摘 要:目的 了解念珠菌性阴道炎患者疾病进展过程中阴道常见真菌分布的变化特点.方法 采用荧光定量PCR技术检测健康女性(H组)、单纯性念珠菌性阴道炎患者(SVC组)、复发性念珠菌性阴道炎患者(RVC组)阴道常见真菌菌种,分析真菌菌种分布与病程发展的关系.结果 RVC组白色念珠菌、光滑念珠菌、近平滑念珠菌和热带念珠菌的真菌数量较SVC组和H组都显著升高(均P<0.001),克柔念珠菌和酿酒酵母菌的数量仅显著高于H组(P<0.05),而与SVC组比较无统计学差异.SVC组白色念珠菌、光滑念珠菌和克柔念珠菌的真菌数量较H组显著升高(均P<0.05).白色念珠菌是各组研究对象阴道标本中检出率最高的菌种.RVC和SVC组白色念珠菌、光滑念珠菌和热带念珠菌的阳性检出率都显著高于H组(均P<0.05),而前两组之间光滑念珠菌和近平滑念珠菌的检出率存在统计学差异(P<0.05).RVC组非白色念珠菌比例较其他两组显著升高(P<0.05).结论 念珠菌性阴道炎患者阴道常见真菌菌群结构发生显著改变,荧光定量PCR技术的应用有助于念珠菌性阴道炎的快速诊断和正确治疗.Objective To investigate the distribution of common vaginal fungi in various stages of vaginal candidiasis. Methods Quantitative real-time PCR was used to detect vaginal common fungi in vaginas of healthy women (H), patients with sporadic vaginal candidiasis(SVC) and patients with recurrent vaginal candidiasis(RVC). The distribution of vaginal fungal species in relation to the disease progression of vaginal candidiasis was analyzed. Results Compared with groups SVC and H, target DNA copies for C. albicans, C. glabrata, C. parapsilosis and C. tropicalis in group RVC were significantly increased (P〈0.001). In group SVC, target DNA copies for C. albicans, C. glabrata and C. krusei were much higher than those in group H(P〈0.05). For C. krusei and S. cerevisiae, no significant difference of target DNA copies was observed between groups RVC and SVC. The prevalence of C. albicans, C. glabrata and C. tropicalis in groups RVC and SVC was significantly higher than those in group H (P〈0.05). Between groups RVC and SVC, significant differences were observed for the prevalence of C. glabrata and C. parap- silosis (P〈0.05), Conclusion The distribution of common vaginal fungi in various stages of vaginal candidiasis is significantly changed and the application of real-time PCR is helpful for rapid diagnosis of vaginal candidiasis.
