严重创伤后的代谢反应与中药治疗的临床研究

摘　　要：为了了解损伤严重程度与生化改变之间的关系以及地鳖虫合剂治疗严重损伤的临床疗效 ,随机将 2 0 0例 AIS-ISS评分法为 16～ 2 4分的病人分成两组进行观察 ,对照组中重伤病人的总蛋白低于中等伤病人 ,甘油三脂、血糖和血沉均高于中等伤病人 ( P<0 .0 1) ,说明创伤后 ,代谢反应增高 ,损伤越严重 ,其代谢反应越剧烈。治疗组总蛋白量在第 3～ 15天内均高于对照组 ( P<0 .0 5 ) ;甘油三脂在第 3～ 10天均低于对照组( P<0 .0 5 ) ;治疗组血糖在第 3～ 2 1天均低于对照组 ( P<0 .0 5 ) ;治疗组血沉在第 3～ 15天均低于对照组 ,第2 1天全部恢复正常 ,对照组仍高于正常 ( P<0 .0 1) ;治疗组第 4天 64 % ,第 6天 94% ,体温恢复正常 ,对照组第 4天 5 % ,第 6天 2 2 %恢复正常。说明中医药治疗骨折创伤疗效肯定 ,具有减轻蛋白分解 ,促进蛋白合成 ,减少脂肪分解 ,降低血糖消耗 ,减少肝糖原分解等作用 。Objective:To evaluate relation between severe trauma and biochemistry change and to study clinic effect of the Eupolyphage sinensis mixture on the severity trauma.Method:According to the AIS-ISS standard,200 cases with 16～24 grade were randomly divided into the treatment group and control group.Result:In control group,different grade of trauma showed different level of total protein,triglycerine,blood sugar,blood sedimentation rate.The severe one was lower total prptein and higher triglycerine,blood sugar,blood sedimentation rate than that medium one.(P<0.01).It confirms that the more severe trauma,the more acute metabolism response.In treatment group,as compared with the control group,during 3～15 days,serum total protein wre showed higher (P<0.05),while triglycerine (3～10 days) lower (P<0.05),blood sugar (3～21 days)lower (P<0.05).The blood sedimentation rate was lower during 3～15 days and normal after 21 days.but the control group still high (P<0.01),64% cases had normal temperature after 4 days, comparing that 5% after 4 days and 22% after 6 days.Conclusion:The Chinese medicinal mixture-Eupolyphage sinensis mixture showed clinic effects on fracture trauma.The mixture could accelerate composition of protein and depress decomposition of protein,fat,liver glucogen.It could also reduce consumption of blood sugar.The basic metabolisms recover quickly.
