摘  要:目的进一步提高骨质疏松症(OP)的诊断水平。方法对20例OP患者(低骨量者11例,骨质疏松者9例)行1H磁共振波谱(1H-MRS)检查,测量脂肪分数(FF)、脂水峰比(LWR)及基线宽度(LW),比较股骨头、股骨颈及股骨大粗隆区各参数值的差异;将结果与20例健康体检骨密度(BMD)检测正常者比较。对各部位1H-MRS分析指标与BMD进行相关性分析。结果骨质疏松者FF和LWR均高于正常者及低骨量者(P均<0.05)。所有研究对象不同感兴趣区的FF和LWR均与整体髋BMD值呈负相关(P均<0.05)。结论 OP患者股骨近端骨髓脂肪含量高于正常人,1H-MRS可通过观察股骨骨髓脂肪含量变化来预测OP的发生;此为评价OP及预防其引发的骨折提供了新思路。Objective To further improve the diagnostic level of osteoporosis (OP). Methods Total of 20 patients with OP ( 11 cases of osteopenia, 9 cases of osteoporosis) were selected to undergo the examination of 1^H magnetic resonance spectroscopy (1^H-MRS). The fat fraction (FF), lipid water ratio (LWR) and 1 ine width (LW) were evaluated. The values of femoral head, femoral neck and great trochanter regions were compared in all objects. All OP patients' results were compared to 20 cases of healthy volunteers with normal bone mineral density ( BMD), correlation analysis between BMD and 1^ H-MRS indices was also proceeded in all three regions. Results The values of FF and LWR were significantly higher in subjects with osteoporosis compared with that in subjects with osteopenia or normal BMD ( all P 〈 0.05 ). There were significant negative correlations between BMD and FF , LWR of different ROI in all groups( all P 〈0.05). Conclusions Bone fat content throughout the proximal femur increased in osteopenic and osteoporotic subjects compared with that in normal subjects, 1 ^H-MRS can be used to predict the occurrence of osteoporosis by observing changes of femoral fat content; this provides a new idea for the evaluation of OP and prevention of osteoporotic fractures.
