补脾益肾、清利化浊法治疗岭南地区CKD2-3期临床疗效观察

摘　　要：目的:评价补脾益肾、清利化浊法配合治疗岭南地区慢性肾脏病2～3期脾肾气虚,湿热内蕴者的临床疗效与安全性。方法:将60例CKD2-3期脾肾气虚,湿热内蕴证患者随机分为试验组(在西医基础治疗基础上加用补脾益肾、清利化浊中药)30例与对照组(仅采用西医基础治疗)30例,连续治疗6个月。观察两组肾功能(eGFR、Scr、BUN)、24h尿蛋白定量(Upr)、血浆白蛋白(Alb)、血红蛋白(Hb)、肝功能(ALT、AST)、电解质(K+、Na+、Cl-、Ca2+)、心电图、肾脏B超及中医症状、体征及症候积分的变化等指标。结果:试验组肾功能、Hb、Alb较治疗前明显好转趋势;两组治疗后24hUpr较治疗前均显著下降;两组肝功能、电解质等指标较治疗前差值无统计学意义;试验组较对照组能够明显减少各症状积分。结论:补脾益肾、清利化浊法可以有效改善岭南地区早中期CKD脾肾气虚,湿热内蕴者患者的肾功能,改善贫血,降低24h尿蛋白定量,改善中医症候,从而提高生活质量,且不良反应轻,具有良好的安全性。Objective： To evaluate the clinical efficacy and safety of the clearing heat and promoting spleen and kidney, eliminating turbid pathogen method（CHPSK method） on chronic kidney disease（CKD） 2-3 with Qi asthenia of both the spleen and kidney, damp in Lingnan region. Methods： 60 patients of CKD2-3 with Qi asthenia of both the spleen and kidney syndromes evidence damp, were randomly divided into two guoups. Patients in experimental group （n=30） were treated the use of CHPSK method based on Western-based treatment. 30 cases in the control group were only accept Western-based treatment, both groups treat continuously for 6 months. The clinical effect included eGFR, Scr, BUN, 24h-Upr, Alb, Hb, safety index as ALT, AST, K＋ ,Na＋,C1 , Ca2~, ECG, renal ultrasound and clinical symptoms, signs and symptoms and changes in indicators of integration were observed. Results： The renal function, Hb, Alb were significantly improved in comparison with that before treatment~ 24h-Upr after treatment was decreased than that before treatment significantly~ liver function, electrolytes and other indicators were not higher than that before treatment in two groups~ test group significantly reduced compared with the control group to the symptom score. Conclusion. The CHPSK method has definite effect on CKD2-3 patients with Qi asthenia of both the spleen and kidney, damp in Lingnan region, and can improve the symptoms of traditional Chinese medicine with good security.
