摘 要:目的:探讨产前超声诊断双胎绒毛膜性的价值。方法:回顾分析我院172例经产后病理证实的双胎绒毛膜性的临床资料,总结其声像图特征。结果:在172例双胎中,其超声图像特点为:首诊孕周≤7孕周均可通过孕囊记数的方法诊断绒毛膜性,诊断符合率为100%,但很难确定羊膜囊性。首诊孕周为8~10孕周可通过记数来判断绒毛膜性和羊膜性,其诊断符合率为100%。首诊孕周为11~14孕周、14~28孕周及>28孕周可通过胎盘的记数、双胎峰征(A)字征、T字征及分隔膜的有无和厚度来判断绒毛膜性,其诊断符合率分别为100%、83.8%、51.9%(P分别>0.05、>0.05、<0.05)。结论:8~10孕周和11~14孕周是超声诊断双胎绒毛膜性的最佳时期。Objective: To evaluate the value of prenatalultrasonic diagnosis of chorionicity in twin pregnancy. Methods: The chorionicity confirmed by postnatal pathology in172 twin pregnancy in our hospital were reviewed and the ultrasonic characters were analysised. Results: When the first ultrasonic diagnosis of gestational week 47 gestational weeks, chorionicity can be diagnosed by gestational sac count via 100% accuracy , however, it is difficult to confirm amniotic sac. When the first diagnosis of gestational week is during 8--10 gestational weeks , chorionic sac and amniotic sac can be counted via 100% accuracy. When the first diagnosis of gestational week is during 11-14 , 14-28, or larger than 28 gestational weeks, chorionicity can be diagnosed hy placenta count, twin-peak-sign, T-sign, partitive membrane's existence and thickness. For those different gestational weeks, the accuracy is 100 %, 83.8%, 51.9% respectively (corresponding P〉 0. 05,P〉0. 05,P〈0. 05). Conclusion: The 8--10 and 11-14 gestational weeks are the best time to determine the chorionicity of twin pregnancy with ultrasonography.
