中西医结合治疗原发性难治性肾病综合征的meta分析

摘　　要：目的:系统评价中西医结合治疗难治性肾病综合征(Refractory Nephritic Syndrome,RNS)的疗效,为中西医结合治RNS提供科学依据。方法:用循证医学的方法,选用国内外多个权威数据库为资料源,对国内近10年间(2001～2010)正式发表的临床研究文献进行手工检索。按照国际cochrane协作网的系统评价方法,对所纳入的研究进行方法学质量评价,并选取11篇质量良好的随机对照试验(RCT)运用Rev.Man5.0.23软件进行Meta分析。结果:共检索到11个符合纳入标准的RCT,共计922例患者,其中中西医结合组478例,对照组444例。Meta分析显示,在治疗后中西医结合组与对照组的总有效率差异有统计学意义[RR=4.04,95%CI(2.85,5.70),合并效益Z=7.93(P<0.00001)]。结论:中西医结合治疗RNS有助于提高其临床疗效,减少激素或免疫抑制剂不良反应的发生。Objective. To evaluate the therapeutic effect of the Chinese and Western Medicine combination in Refractory Nephritic Syndrome （RNS）, for providing a scientific basis in combined Chinese and Western Medicine in RNS. Methods. To search the clinical research literature which officially published nearly ten years （ 200142010 ） in a number of domestic and international authoritative database, By using the evidence~ based medicine method. Then take methodological quality assessment for the included studies in accordance with international Cochrane network system assessment method. Meta-analysis was Performed using Rev. ManS. 0. 23 software for 11 best quality randomized controlled trials （ RCT ）. Results： Searching 11 the standard RCTs, a total of 922 patients, including 478 cases of combination of traditional Chinese and West- ern medicine group, 444 cases in the control group. Meta analysis showed that, after treatment with inte- grated traditional Chinese and Western medicine group and control group in the total efficiency difference has statistics significance [RR=4.04, 95%CI （ 2.85, 5.70）, the combined benefit of Z=7.93 （P^0. 00001）]. Conclusion. It is contributes to improve clinical efficacy, take reduction of corticosteroid or immunosuppres- sive adverse reaction for combined Chinese medicine with western medicine in treatment of RNS.
