盐酸戊乙奎醚和阿托品治疗有机磷农药中毒疗效对比观察

摘　　要：目的观察盐酸戊乙奎醚(长托宁)肌肉注射治疗有机磷农药中毒的临床效果。方法将112例有机磷农药中毒患者随机分成两组,治疗组采用盐酸戊乙奎醚肌注,对照组采用阿托品静推,其他如清洗皮肤、洗胃、导泻、灌肠、血液灌流、大量补液及对症治疗均相同。结果 M样症状消失时间,治疗组:轻度0.5h,中度1～1.5h,重度2～3h;对照组:轻度3～4.5h,中度4.5～6h,重度6～7.5h。胆碱酯酶恢复60%的时间,治疗组:轻度16～20h,中度20～24h,重度24～28h;对照组:轻度26～32h,中度32～44h,重度44～52h。药物不良反应,治疗组较对照组发生率低。治疗组总有效率97.3%,对照组总有效率88.2%。病员在院时间,治疗组3～6天,对照组7～14天。两组给药的量和给药次数、阿托品化后的不良反应及治愈的时间上差异均有统计学意义(P<0.05)。结论盐酸戊乙奎醚治疗有机磷农药中毒安全有效,便捷,疗效优于阿托品。Objective To observe the clinical efficacy of intramuscular injection of penequinine hydrochloride in the treatment of organic pesticide poisoning. Methods 112 cases of organic pesticide poisoning were randomized into the observation group treated with penequinine hydrochloride, and the control group, treated with atropine. The two groups both received symptomatic and supplementary treatments such as skin cleaning, gastric lavage, catharsis, clysis, blood perfusion and fluid infusion. Results M type symptom vanishing time was respectively （mild, 0.5h; moderate, 1-1.5h; severe, 2-3h） in the observation group and （mild, 3-4. 5h; moderate, 4.5-6h; severe, 6-7.5h） in the control group. 60% of cholinesterase recovery time was respectively （mild, 16-20h; moderate, 20-24h; severe, 24-28h） in the observation group and （mild, 26-32h; moderate, 32-44h; severe, 44-52h;） in the control group. The incidence of adverse drug reactions in the observation group was lower than that in the control group. The effective rate was 97.3% in the observation group and 88.2% in the control group. Hospital stay was 3-6d in the observation group and 7-14d in the control group. Two set of L dosing quantity and frequency of administration of atropine, adverse reaction and cure time difference has statistics significance（P〈0, 05）. Conclusion Penequinine hydrochloride is a safe, effective and convenient drug with a better efficacy than atropine in the treatment of organic pesticide poisoning.
