摘　　要：[目的]总结痔上黏膜环切术2次切割吻合治疗Ⅳ期混合痔的手术配合。[方法]13例Ⅳ期混合痔病人行痔上黏膜环切术(PPH)2次切割吻合治疗,同时加强手术配合及护理。[结果]13例病人均顺利手术,手术平均时间37min,病人再次切割吻合成功,无大出血、大便失禁、吻合口漏等并发症,两吻合口之间无缺血现象;术后短期尿潴留3例,肛门水肿、疼痛5例;术后随访3周至10个月,无复发及吻合口狭窄等。[结论]加强痔上黏膜环切术2次切割吻合治疗Ⅳ期混合痔的手术配合,是手术成功的保证。Objective:To summarize the operative coordination of PPH with twice cutting anastomosis for treatment of stage Ⅳ mixed hemorrhoids.Methods:A total of 13 cases of patients with stage Ⅳ hemorrhoids accepted PPH with twice cutting anastomosis.At the same time,the operative coordination and nursing care were strengthened.Results:All patients had successful operations.The average operation time was 37min and there were no any complications such as bleeding,fecal incontinence,anastomotic leak and so on.There were 3 cases of urinary retention after the operation,5 cases of anal edema and pain.But there was no recurrence and anastomotic stricture after the operation followed up for 3 weeks to 10 months.Conclusion:To strengthen the operative coordination of PPH with twice cutting anastomosis for the treatment of stage Ⅳ mixed hemorrhoids is the guarantee of successful operations.
