针刺对神经源性膀胱储尿功能影响的穴位特异性研究

摘　　要：目的探讨针刺中极穴对神经源性膀胱充盈期尿动力学影响的即时效应,以优化临床选穴。方法将61例神经源性膀胱患者随机分为中极组和外关组,分别针刺中极穴和外关穴;检测治疗前后尿动力学变化情况。结果与治疗前比较,中极组初尿感容量(FDV)、初次急迫膀胱容量(FUV)、强烈急迫膀胱容量(SUV)、最大膀胱测量容量(MCBC)均明显升高(P<0.05),最大逼尿肌压力(Pdetmax)明显降低(P<0.05);但外关组上述指标无明显变化(P>0.05)。两组ΔFDV、ΔFUV、ΔSUV、ΔMCBC有显著性差异(P<0.05,P<0.01)。结论中极穴对神经源性膀胱储尿功能的调整作用具有特异性。Objective To evaluate the transient effects of acupuncture at aeupoint Zhongji（CV3） on urodynamics in neurogenic bladder（NB） patients. Methods Sixty-one subjects were randomized into observation group and control group, and respectively treated by needling Zhongji（ CV3 ） and Waiguan （TE5）. The uro- dynamic test was taken before and after acupuncture. Results After acupuncture, the FDV, FUV, SUV and MCBC were increased markedly（ P 〈 0.05 ） , and the Pdetmax was lowered apparently （ P 〈 0.05 ） in the observation group ; but they did not change in the control group（ P 〉 0.05 ）. There were striking differenees in the △FDV, △FUV, △SUV and △MCBC between the two groups （ P 〈 0.05, P 〈 0.01 ）. Conclusion Aeupoint Zhongji （ CV3 ） has specific regulative effeets on the urine storage of neurogenic bladder.
