4例心肌淀粉样变的超声心动图和心电图特点及文献复习

摘　　要：目的：总结心肌淀粉样变的超声心动图和心电图的临床特点,以提高临床认识。方法：回顾分析2005~2011年于我院就诊并经病理证实的4例心肌淀粉样变患者的资料,分析其超声心动图和心电图的特点,并进行文献复习。结果：4例超声心动图中,发现左室室壁向心性肥厚（4例）、右室肥厚（2例）、房间隔增厚（2例）、心房扩大（2例）、心肌点状强回声（3例）、舒张功能障碍（2例）及心包积液（3例）;4例心电图中,发现肢导低电压（4例）及假性梗死波形（2例）,合并多种心律失常。与文献复习结果一致。结论：超声心动图与心电图结果联合分析有助于提高心肌淀粉样变的诊断率。Objective：To summarize the echocardiography and electrocardiography features on patients with cardiac amloidosis.Methods：We retrospectively analyzed patients with cardiac amloidosis for their echocardiographic and electrocardiographic features,and had literature reviewed.All patients were admitted and treated in our hospital from 2005 to 2011.All of them diagnosed as amloidosis by biopsy.Results：All 4 echocardiography presented concentric left ventricular hypertrophy,right ventricular hypertrophy（2 cases）,atrial septum hypertrophy（2 cases）,atrial enlargement（2 cases）,granular sparking appearance of the myocardium（3 cases）,ventricular diastolic dysfunction（2 cases）and pericardial effusion（3 cases）.All the voltage of the limb leads were low.Electrocardiography presented pseudo-infarction patterns（2 cases）combined with various kinds of arrhythmia as well as reported in literature review.Conclusion：Combining analysis of the echocardiography and electrocardiography is helpful for improving the diagnosis rate of cardiac amloidosis.
