摘 要:目的探讨足月新生儿脐血脂联素(APN)水平变化及其对新生儿发展趋势的影响。方法研究对象为大于胎龄儿(LGA)、适于胎龄儿(AGA)、小于胎龄儿(SGA)各30例。采用放射免疫分析方法测定脐血和母血脂联素水平。采用免疫比浊法测定三酰甘油(TG)、总胆固醇(TCH)、低密度脂蛋白(LDL-C)、高密度脂蛋白(HDL-C)水平。并分析脐血脂联素水平与母血脂联素、体质指数(BMI)、胎盘重量和血脂水平的相关性。结果 LGA脐血脂联素水平明显低于AGA,差异有显著性(P=0.002);SGA脐血脂联素水平稍低于AGA,但差异无统计学意义(P=1.000);LGA及AGA血TG、TCH、LDL-c、HDL-c水平与AGA比较:除LGA血TCH明显低于AGA差异显著(P=0.000)之外,其余两两比较差异无统计学意义(P>0.05)。LGA脐血脂联素水平与新生儿BMI、胎盘重量、脐血TG呈显著负相关(r=-0.848、-0.780、-0.783,P<0.01);SGA脐血脂联素水平与新生儿身长、新生儿BMI、胎盘重量呈正相关,但与脐血TG呈负相关(r=-0.231,P=0.026)(r=0.386、0.431、0.365,P>0.05)。脐血脂联素水平与母血脂联素,孕前及分娩时产妇BMI无相关性(P>0.05)。结论脐血脂联素水平变化与LGA约发生有关,测定脐血脂联素水平有助于判断LGA约发展趋势。Objective To explore the clinical significance of changes in cord plasma levels of adiponecfin in newborns. Methods In this study, 30 infants with small gestafional age ( SGA ) and 30 infants with appropriate gestational age (AGA) and 30 infants with birth weight large gestational age (LGA) were enrolled in this study and cord plasma level of adiponectin was determined by radioimmunoassay (RIA) and the correlation between cord and puerpera plasma levels of adiponectin was compared, maternal plasma level of adiponecfin, gender and birth weight of newborns, body mass index (BMI), placental weight and blood level of lipids were analyzed. Serum levels of triglyceride (TG), total cholesterol (TC), low -density lipopmtein - cholesterol (LDL- C) and high- density lipoprotein- cholesterol (HDL- C ) were assayed by turbidimetry. Results Compared with A GA group, cord plasma level of adiponectin in LGA infants was significantly decreased ( P =0. 002), and plasma levels of TG, LDL - C and I-IDL - C were slightly changed ( P 〉 0.135), and plasma level of TC was significantly decreased ( P =0.002). Compared with A GA group, cord plasma level of adiponectin in SGA infants was slightly changed ( P 〉0.05), and plasma levels of TG, TC, LDL -C and HDL -C were also slightly changed ( P 〉0.05). The level of adiponectin in LGA infants had negative correlation with birth weight of newborns, BM/, placental weight and cord blood level of TG ( r = -0.848, -0.780, -0.783, P 〈0.01). The level of adiponectin in SGA infants had positive correlation with length of infants, birth weight of newborns, length and placental weight ( r =0.386, 0.431,0.365, P 〈0.05). There was negative correlation with cord blood level OfTG ( r = -0.231, P =0.026). The cord plasma adiponectin level had no correlation with plasma level of adiponectin in puerpera.
