常规MRI对肩关节损伤的诊断价值

摘　　要：目的分析常规MRI检查对肩关节损伤的诊断价值。方法回顾分析18例肩关节损伤患者的常规MRI表现,其中有外伤史5例,无外伤史13例,均有肩关节疼痛,患肢活动受限。结果 15例肩袖损伤中,10例肩袖部分撕裂,表现为局部高信号且累及肌腱关节面;5例显示完全撕裂,表现为高信号贯穿肌腱全层,肌腱不连续,断端回缩松弛,三角肌下脂肪线消失,滑囊积液;盂唇损伤3例,关节囊损伤13例,肱骨头损伤5例,反复盂肱关节脱位1例,半脱位1例。结论 MR检查能够准确显示肩关节常见病变及相关组织的病理发展过程,为临床诊断及治疗提供影像学依据。Objective Investigate diagnosis value of normal magnetic resonance imaging （MRI） about injury of shoulder. Methods Review radiological materials about 18 cases by normal MRI scan. In them,15 cases are rotator cuff injury, 3 cases are gleno- humeral joint instability, and all of them have pain. Results In 15 cases about rotator cuff injury, 10 cases have rotator cuff tear （showing high signal involved tendon and articular surface partially）, 4 cases tear completely （showing high signal across tendon and the sign of fat line under deltoid are disappeared, bursae hydrops）, 1 case is missed diagnosis. In another side, 3 cases are glenoid labrum injury, 13 cases are joint capsule injury and 5 cases are head of humerus injury. Conclusion Normal MRI scan can show the signs of shoulder injury exactly, it also can provide imaging evidence for clinical diagnosis and treatment.
