儿童慢性鼻-鼻窦炎三阶梯治疗分析

摘　　要：目的探讨儿童慢性鼻一鼻窦炎（CRS）的三阶梯治疗方式与疗效。方法对100例儿童CRS患者的临床资料进行分析。采用三阶梯治疗方式：一，系统性药物治疗；二，非鼻窦手术；三，鼻内镜鼻窦手术。结果随访12～18个月，系统性药物治疗42例，治愈30例（71％），好转9例（21％），无效3例（8％）；非鼻窦手术治疗35例，治愈28例（80％），好转5例（14％），无效2例（6％）；鼻内镜鼻窦手术治疗23例，治愈17例（74％），好转4例（17％），无效2例（9％）；总体治愈75例，好转18例，总有效率为93％。结论正确选择治疗适应证，采用三阶梯方式治疗儿童CRS可取得较好疗效。Objective To investigate the method and curative effect of three stepwise treatment for pediatric patients with chronic rhinosinusitis（CRS）. Methods The clinical data of 100 children with chronic sinusitis were retrospectively analyzed. Firstly, expectant treatment systemically; Secondly, aux- iliary surgery;Thirdly, endoscopic sinus surgery. Results All patients were followed up for 12 -18 months. Forty-two cases were treated systemically with medicine, 30 cases （71%） were cured, 9 cases （21%） improved and 3 cases（g% ） showed no effect. Thirty-five cases were treated with auxiliary sur- ge,7, 28 eases（80% ） were cured, 5 eases（14% ） improved and 2 eases（6% ） showed no effect. Twen- ty-three eases were treated with endoscopic sinus surgery, 17 cases（74% ） were cured, 4 cases（ 17% ） improved and 2 eases （9%） showed no effeet. The overall efficiency was 93%. Conclusions Three stepwise treatment for chronic rhinosinusitis is effective in children.
