生长抑素受体显像在胰岛素瘤诊断中的价值

摘　　要：目的评价^(99m)Tc-HYNIC-TOC生长抑素受体显像诊断和鉴别诊断胰岛素瘤的临床价值。方法回顾性分析近11年来65例胰岛素瘤患者的^(99m)Tc-HYNIC-TOC显像结果 ,并与胰腺灌注CT、B超或内镜超声结果比较。结果 65例患者均进行^(99m)Tc-HYNIC-TOC显像,阳性28例,检出率43.1%。部分患者进行胰腺灌注CT、B超或内镜超声检查,检出率分别为92.5%、25.6%和76.9%。结论单独应用^(99m)Tc-HYNIC-TOC显像对胰岛素瘤的定性诊断和鉴别诊断临床价值有限,需联合应用多项分子影像诊断技术。Objective To assess the differential diagnostic value of somatostatin receptor imaging with 99mTc-HYNIC- TOC （99mTc-hydrazinon-ieotinyl-Tyr3-oetreotide） in insulinoma. Methods The results of 99mTc-HYNIC-TOC imaging in 65 patients with insulinoma were reviewed and compared with CT, ultrasound and endoscopic uhrasonography. Results Positive findings were obtained in 28 of 65 patients, the localization rate of 99mTc-HYNIC-TOC imaging for insulinoma was 43.1%. Accurate localization rate of CT, ultrasound and endoscopic uhrasonography was 92.5% ,25.6% and 76.9% respectively. Conclusion 99mTc-HYNIC-TOC imaging is limited in diagnizing in sulinoma and should be combined with other imaging methods.
