二尖瓣腱索断裂的超声心动图诊断与临床分析

摘　　要：目的探讨二尖瓣腱索断裂的超声特征及病因。方法对彩色多普勒超声心动图诊断的47例二尖瓣腱索断裂患者的临床资料、病因、超声检查所见进行分析。结果①所有病例均可见二尖瓣活动异常、收缩期瓣缘对合不良及不同程度二尖瓣返流;左房、左室扩大;后叶腱索断裂多于前叶腱索断裂;②二尖瓣腱索断裂病因多样,19例有心脏病基础,见于风湿性心脏病、冠状动脉粥样硬化性心脏病、先天性心脏病及高血压性心脏病等。无心脏病基础的自发性二尖瓣腱索断裂,可能部分与二尖瓣装置先天性发育不良有关。此外,老年患者居多,考虑与瓣膜、腱索退行性病变有关。结论彩色多普勒超声心动图可以明确腱索断裂的部位及程度,是其它检查方法不可替代的。同时,可以明确其心脏病性质,对二尖瓣腱索断裂病因做出判断。Objective To investigate the sonographic features and etiology of the mitral valve chordae rupture. Methods The clinical data, cause, ultrasound examination findings of 47 cases of such patients, who were diagnosed by color Doppler echocardiography, were analyzed. Results （1） In all cases were found mitral activities exception, the systolic valve edge co - adverse and varying degrees of mitral regurgitation ; left atrium, left ventricular enlargement; posterior lobe chordae rupture than the anterior lobe chordae rupture; （2）Diverse causes of mitral chordae rupture, 19 cases have heart disease basis of rheumatic heart disease, coronary atherosclerotic heart disease, congenital heart disease and hypertensive heart disease, et al. Without heart disease based on spontaneous mitral valve chordae rupture may be part of the mitral valve congenital dysplasia, in addition to the mostly elderly patients were considered that maybe have relativity of valves and Chordae Tendineae degenerative chan- ges. Conclusion Color Doppler echocardiography can identify the location and degree of chordal rupture. It can- not be replaced by other examination methods ; It also can identify the nature of their heart disease and make judgments the etiology of the mitral valve chordae fracture.
