妻子对丈夫免疫耐受基础研究

摘　　要：目的:探讨妻子对丈夫免疫耐受的机制,为丈夫给妻子供肾提供理论支持。方法:选取100对已婚夫妻,按婚龄分组,每5年为一组,每组20名。ELISA方法检测外周血中IL-4,IL-10,TNF-α,INF-γ水平;PCR-STR检测Y染色体嵌合体;并进行淋巴毒试验。结果:各组IL-4,IL-10,TNF-α,INF-γ水平测得值均在正常范围,各组间的差异无显著性。各组Y染色体嵌合体阳性率差别有显著性(X2=9.856,P=0.027),进一步行两两比较的X2检验显示,以0～5年、6～10年两组的阳性率最高,与11～15年组组间的差异无显著性,与15～20年、>20年差别有显著性。各组淋巴细胞毒测得值均在正常范围,各组间的差异无显著性,但随着婚龄的延长,淋巴毒有下降的趋势。结论:婚龄在前10年有过生育史的夫妻中,妻子体内存在Y染色体的嵌合体发生率较高;随着生活时间的延长,夫妻间淋巴细胞毒测定值有降低的趋势,这也与早期的免疫耐受形成有关,说明妻子对丈夫存在一定程度的免疫耐受。Objective： To explore the mecha- nisms of immune tolerance from wife to her husband, in order to provide theoretical sup- port for the kidney transplantation between couples that donor is the husband. Methods： To choose 100 pairs of married couples grouped with marriage age, every five years for a group （n --20 ） , measured by ELISA in peripheral blood of IL -4, IL - 10, TNF - a,INF-y level; PCR -STR testing the Y chromosomal chimaera; and proceeding the lymphocytotoxicity cross - match test. Re- suits： The IL - 4, IL - 10, TNF - a, INF - y values of each group were within normal limits, with no significant difference. Howev- er, there was significant differences in the positive rate of the Y chromosomal chimaera （X2=9. 856, P = 0. 027 ）, further using the chi - square test to pairwise comparison ： The highest positive rate were both the 0 -5 years and 6 - 10 years groups,which was no significant difference with the 11 - 15 years group, however, there was significant difference with both the 15 -20 years group and more than 20 years groups. Meanwhile, the lymphocytotoxicity measured value was in the normal range, the differences between the groups were no significant, but with the extension of marriageable age, the lymphocy-totoxicity measured value was downward trend. Conclusion： There＇ re higher inci- dence of Y chromosome chimerism in the wife＇s body when the couples whose marr- ying age is less than 10 years and who have had childbearing history;With the extension of time of life, the lymphocytotoxicity meas- ured value is decreased between husband and wife, which is related with the formation of early immune tolerance, and this just explained that there is a certain degree of immune tolerance from wife to her husband.
