择期剖宫产与第二产程剖宫产对产后早期盆底功能的影响

摘　　要：目的:探讨不同阶段剖宫产对产后早期盆底功能的影响。方法:随机抽取2010年5月～2011年3月剖宫产分娩的初产妇41例,其中选择性剖宫产24例,第二产程剖宫产17例。分别于产后6～8周及12～14周进行POP-Q评分、尿垫试验、超声检测残余尿,比较两组产妇压力性尿失禁(SUI)、盆腔器官脱垂(POP)的发生率。结果:产后6～8周和产后12～14周随访时,SUI发生率依次为选择性剖宫产组4.16%(1/24),0(0/24);第二产程剖宫产组23.52%(4/17),29.41%(5/17),两组比较差异均无统计学意义(P>0.05)。产后6～8周和产后12～14周随访时,POP发生率依次为选择性剖宫产组37.5%(9/24),33.33%(8/24),较第二产程剖宫产组70.58%(12/17),76.47%(13/17)低,差异均有统计学意义(P<0.05)。结论:选择性剖宫产可降低产后早期POP的发生率,并可减少发生SUI的风险。Objective To investigate the effect of different periods of cesarean section on early postpartum pelvic floor function. Method The 41 primary pregnant women who had their childbirth after cesarean section in Gongming Republic Hospital from May 2010 to Mar 2011 were randomly selected and were divided into two groups, elective cesarean section group and cesarean section in the second stage of labor group. In 6 - 8 weeks and 12 -14 weeks after childbirth, they were respectively given POP -Q scoring, protective pad experimenting and ultrasonic testing of residual urine. An analysis on their occurrence of SUI and POP was carried out. Results Followed up of the pregnant women in 6 -8 weeks and 12 - 14 weeks after their childbirth, the proportion of occurrence of stress urinary incontinence was respectively 4. 16% （1/24） ,0 （0/24） in elective cesarean section group and 23.52% （4/17） ,29. 41% （5/17） in cesarean section in the second stage of labor group. There was no significant difference between the two groups （ P 〉 0. 05 ）. The proportion of occurrence of pelvic organ pro- lapse was respectivly 37.5% （9/24） ,33.33% （8/24） in elective cesarean section group,lower than that was 70. 58% （12/17） ,76. 47% （13/17） in cesarean section in second stage of labor group,and the all differences have statistical significance（P 〈0. 05）. Conclusion In comparison,elective cesarean section can reduce the impact on pelvic floor function.
