自体颗粒脂肪移植重塑面部轮廓并改善皮肤质地

摘　　要：目的:观察自体颗粒脂肪移植重塑面部轮廓的疗效,探讨脂肪干细胞(adipose-derived stem cells,ADSCs)改善皮肤质地的作用。方法:选取37例先天或后天因素所致颞部、眶周、颊部凹陷患者,抽取腰腹部、臀部或大腿部脂肪颗粒,静置纯化后,于凹陷区域进行多隧道、多层次注射。每例脂肪移植1～3次,间隔3～6个月。通过比较术前及术后的照片差异、脂肪吸收程度、皮肤质地变化和患者自我感觉等指标,评价治疗效果。结果:所有病例均随访6个月以上,最长2年。其中17例颊部、9例眶周凹陷均在1次注射后取得满意疗效;4例眶周、6例颞部凹陷2～3次注射后基本满意;1例颞部凹陷3次注射后仍不满意。另外,19例脂肪移植后表面皮肤的色泽、弹性明显改善;13例稍有改善;5例变化不明显。结论:自体颗粒脂肪移植的疗效不仅取决于脂肪收集、纯化、注射方法,而且还受病人体质,手术部位等因素的影响。脂肪干细胞对改善皮肤质地起到一定作用。Objective To observe clinical effect of autologous fat grafting for facial contour remodeling and to discuss the role of adipose-derived stem cells for skin texture improving. Methods Autotogous fat granule was harvested from patient＇s abdomen, flanks and thighs respectively. After standing for stratification refinement, fat granule was grafted into temple, periorbital and cheek depression regions by multi-tunnel,multi-plane injection. 37 cases were included. Each case was treated 1-3 times and the interval period is 3 ~6 months. Efficacy was evaluated by contrast of photo difference,fat absorption degree, skin texture change and patient self-evaluation between pre-operation and post- operation. Results All cases were followed up beyond 6 months and the longest period was 2 years.17 cheek and 9 periorbital cases obtained satisfactory effects after once injection.4 periorbital and 6 temple cases were fundamentally satisfied after 2-3 times injection. A temple case was still disappointed after 3 tJmes injection. Moreover,19 patients felt that the obvious elevation of skin color and elasticity,13 patients a little improvement and 5 patients no change. Conclusion Efficacy of autologous fat grafting depends not only on the procedures of harvesting, refinement and grafting, but also on patient＇s physical condition,surgical operation areas,etc. Adipose-derived stem cells help on improvement of skin texture.
