摘  要:目的以冠状动脉造影(CCA)作对照,应用受试者操作特征(ROC)曲线分析方法评价心电编辑技术在320排CT冠脉成像(CTCA)中的应用价值。方法 77例受检者根据CTCA图像质量分为心电编辑组21例,对照组56例。1-3分评价图像质量的高低。两组间基本资料、图像质量的比较采用Independent-Samplest检验。心电编辑组重建前后图像质量的比较采用Paired-Samplest检验。频数的比较采用Chi-square检验。以CCA作为金标准,绘制两组的ROC曲线并比较其曲线下面积(AUC)。结果心电编辑前,心电编辑组冠脉节段图像的评分(1.261±0.534)高于对照组(1.161±0.368),差异具有统计学意义(t=2.901,P=0.004)。心电编辑后,心电编辑组图像评分(1.192±0.434)高于对照组(1.161±0.368),差异无统计学意义(t=1.056,P=0.291)。心电编辑前后图像评分降低,差异具有统计学意义(t=3.732,P=0.000);3分的冠脉节段所占比例显著减少(χ2=4.263,P=0.039);心电编辑前后、图像质量仍为3分的5个冠脉节段分别为RCA2(40%2/5)、LCX13(20%1/5)、LCX15(20%1/5)、LAD7(20%1/5)。心电编辑组和对照组的AUC值分别为0.976和0.962。结论心电编辑技术在有效改善320CTCA成像质量的同时可提高诊断冠脉狭窄的准确性。Objective To assess the application value of ECG editing technique in 320 slice CT coronary angiography(CTCA) by receiver operating characteristic (ROC) curve analysis, and the results were compared to catheter coronary angiography (CCA). Methods 77 patients were divided into ECG editing group(n=21) and Control group(n=56) by image quality of CCTA. Data in each of the two groups of patient characteristics and image quality were compared using Independent-Samples t test. Image quality of ECG editing group before/after ECG editing were compared using Paired-Samples t test .Frequencies were compared using Chi-square analysis.Area under the receiver operating characteristic curve(AUC) was used to compare diagnostic efficacy in two groups. Results Before ECG editing, image quality scored ( 1.261 ± 0.534 ) of ECG editing group was higher than Control group ( 1.161 ± 0.368 ) ( t=2.901, P=0.004 ). After ECG editing, image quality scored ( 1.192 ± 0.434) of ECG editing group was lower than before ( 1.261 ± 0.534) ( t=3.732, P=0.000 ); Frequencies of segments image quality scored 3 get fewert O2=4. 263, P=0.039 ): image quality scored ( 1.192 ± 0.434 ) of ECG editing group was little higher than Control group( 1.161 ± 0.368 )( t= 1.056, P=0.291 1. RCA2(40% 2/5), LCX 13 (20% 1/5), LCX15(20% 1/5), LAD7(20% 1/5) were still scored 3 before and after ECG editing. The AUC in ECG editing group and Control group were 0.962 and 0.976 respectively. Conclusion ECG editing technique could improve not only the image quality of 320 slice CT coronary angiography ,but also the diagnostic accuracy of stenosis of coronary artery.
