MSCTA多平面重组及三维重建图像对胡桃夹综合征的诊断价值

摘　　要：目的研究MSCT的血管多平面重组及三维重建图像对胡桃夹综合征的诊断价值。方法应用MSCTA对15例胡桃夹综合征病例进行泌尿系增强多期扫描,并获得各种后处理图像。结果 15例胡桃夹综合征病例均可见肠系膜上动脉(SMA)压迫左肾静脉(LRV)的征象。CT多平面重建(MPR)直观地显示了腹主动脉(AO)与SMA之间的夹角,两者之间交角13.4°～31.8°,平均为21.5°±4.6°。LRV受SMA压迫狭窄处前后径为2.8～4.1mm,平均为3.4mm,近肾端左肾静脉明显扩张,前后径为11.8～18.2mm,平均为15.6mm,两者的管径之比为1:4.20～6.43;受压处左肾静脉上下径明显拉长,直径为11.3～21.2mm。结论 MSCTA可清楚地显示胡桃夹综合征的LRV形态及立体走向,利用重建功能准确、直观地评估LRV扩张及变窄的程度,对临床诊断、治疗具有重要的应用价值。Objective To evaluate the value of multiplanar reconstruction and three dimensional reconstruction images of vessels by using MSCTA in diagnosing nutcracker syndrome. Methods Multi-phase enhancement scanning of urological system were performed to 15 cases with nutcracker syndrome by MSCT scanner, and kinds of post-processing images were acquired. Results The sign of superior mensenteric artery （SMA） compressing left renal vein （LRV） was showed in all cases. The included angle between abodorninal aorta （AO） and SMA could be showed directly by multiplanar reformation. The mean included angle was 21.5° -＋ 4.6° . The anteroposlerior diameter of the stenosis where compressed by SMA was 2.8 i〈〈4. 1mm, with mean value 3.4mm. LRV close to kidney dilatated, and the anteroposterior diameter and mean value of dilated LRV were 11.8 - 18.2mm and 15.6 mm respectively. The diameter of the narrow LRV and dilalaled SMA raiio was l 4.20 - 6.43. LRV was enlongated, and its diameter was 11.8 - 18.2mm. Conclusion MSCTA can shrew LRV of nutcracker syndrome patients clearly, and evaluate the narrow and dilate degree of LRV accurately. MSCTA has important application value in clinical diagnosis and the therapy.
