摘  要:目的观察临床短期应用胰岛素治疗糖尿病脑病的疗效,初步探讨糖尿病脑病的发病机理。方法 80例糖尿病脑病患者随机分为口服降糖药组和胰岛素治疗组,两组均予以饮食控制及运动疗法作为基础治疗,胰岛素治疗组皮下注射胰岛素,口服降糖药组口服阿卡波糖和或二甲双胍,均以3个月为一疗程。结果口服降糖药组治疗后的MMSE评分比较无明显变化(P>0.05),但胰岛素治疗组治疗后的MMSE评分与治疗前比较差异有统计学意义(P<0.05),两组治疗后的MMSE评分比较也有统计学意义(P<0.05)。结论临床短期应用胰岛素治疗糖尿病不仅能有效控制血糖,而且能明显改善糖尿病脑病患者的认知功能,提示胰岛素对改善患者神经细胞功能具有一定的作用。Objective To observe the curative effect of short-term application of insulin on diabetic encephalop- athy and to tentatively explore the pathological mechanism of diabetic encephalopathy. Methods 80 diabetics were randomly divided into the oral administration group and the insulin group. All the patients had food control and exer- cise as the basic treatments. The insulin group was treated by hypodermic insulin injection and the oral administration group took medicine of Acarbose and/or Metformin. Both groups took 3 months as one course of treatment. Results There was no significant difference in the MMSE scores of the oral administration group before and after treatment( P 〉0.05). However,the insulin group showed significant difference in the MMSE group after treatment compared to those before treatment( P 〈0.05). And after treatment, there is statistical difference in MMSE scores between two groups( P 〈0.05). Conclusion Short-term clinical application of insulin could not only control the blood glucose, but also improve the cognition of patients with diabetic eneephalopathy. It is also suggested that insulin may probably have some nutritional effect on neurons of patients.
