739例心脏瓣膜手术患者住院时间及费用分析

摘　　要：目的分析心脏瓣膜手术患者住院时间及医疗费用,为医院管理者和医师压缩术前住院天数、控制住院总费用提供依据。方法统计某心脏病医院739例心脏瓣膜手术患者的平均住院天数及平均住院总费用,并分析比较不同手术方式、不同年龄组、不同瓣膜、不同性别患者等的差异。结果本组739例平均住院(22.5±8.8)d,瓣膜成形术患者住院时间最短为(19.1±6.7)d,瓣膜置换合并冠状动脉搭桥手术住院时间最长为(35.1±17.1)d;心脏瓣膜手术患者住院总费用为(6.35±4.22)万元,进口瓣置换明显比国产瓣置换费用高,差异有统计学意义(P<0.01),住院总费用最高是瓣膜置换+冠状动脉搭桥和大血管+瓣膜置换;不同性别患者住院时间、住院费用比较差异无统计学意义(P>0.05);不同年龄患者住院时间、住院费用比较差异有统计学意义(P<0.01)。结论心脏瓣膜置换术患者住院时间长,住院费用较高,以瓣膜置换+冠状动脉搭桥和瓣膜置换+大血管更明显;年龄对患者住院时间、住院费用有影响。Objective To analyze heart valve operation patients hospitalization days and medical expense to provide references for hospital managers and doctors to shorten preoperative stay length,control total hospitalization cost.Methods Average hospitalization days and total cost of hospitalization of 739 cases of heart valve operation were summarized,and analysis and comparison of different operation modes,different age groups,different valves,gender differences were done.Results The average length of stay was（22.5±8.8）days,the shortest time for valvuloplasty was（19.1±6.7）days;the longest time of valve replacement combined with coronary artery bypass grafting operation was（35.1±17.1）days;total hospitalization cost was RMB（63.5±42.2）thousand Yuan,with the imported valves cost higher than the domestic produced ones,difference with statistical significance（P0.05）;but saw statistical differences between different ages（P〈0.01）.Conclusion Heart valve replacement surgery is with longer hospital stay,higher costs of hospitalization,with valve replacement＋CABG and valve replacement＋vascular operation is more obvious;age has influence on hospitalization time and cost.
