摘  要:目的 探讨细胞粘附分子 ( CAMs)与急性心肌缺血的关系。 方法 采用酶联免疫吸附法 ( ELISA)检测了 4 0例健康人 (对照组 )和 52例不稳定性心绞痛 ( UA,自发性心绞痛 1 8例 ,心肌梗死后心绞痛 2 2例 ,恶化劳力性心绞痛 1 2例 )患者心绞痛发作时和缓解后 2 4 h外周血白细胞 CD1 8表达、血浆可溶性细胞间粘附分子 -1 ( s ICAM-1 )和可溶性血管细胞间粘附分子 -1 ( s VCAM-1 )浓度的变化。 结果  ( 1 ) UA患者白细胞 CD1 8表达、s ICAM-1和 s VCAM-1浓度〔( 6.1 3± 0 .4 8)、( 4.1 0±0 .3 2 )、( 540± 4 5)μg/ L、( 3 87± 3 8)μg/ L和 ( 968± 4 2 )μg/ L、( 72 3± 4 0 )μg/ L〕较对照组〔( 2 .68±0 .2 5)、( 1 64± 2 2 ) μg/ L和 ( 567± 3 6μg/ L)〕明显增高 ( P<0 .0 0 1 ) ;( 2 )心绞痛发作时白细胞 CD1 8表达、s ICAM-1和 s VCAM-1浓度增高较缓解后更明显 ( P<0 .0 0 1 ) ;( 3 )白细胞 CD1 8表达与 SICAM-1、s VCAM-1呈正相关 ( r=0 .568,0 .62 4 ,0 .669,0 .74 6,P<0 .0 0 1 ) ;( 4)不同类型的心绞痛发作时和缓解后白细胞 CD1 8表达、s ICAM-1和 s VCAM-1浓度差异亦有显著性 ( F=1 4 0 .1 4 6,2 2 8.63 3 ,4 40 .93 6,1 1 8.3 99,1 2 8.4 68,1 1 3 .84 1 ,q=1 6.2 56,2 2 .82 8,9.Objective To explore the correlation of cellular adhesion molecules (CAMs) with acute myocardial ischemia. Methods By means of enzyme linked immunosorbent assay (ELISA), the changes of leukocyte CD18 expression in peripheral blood and the concentrations of plasma soluble intercellular adhesion molecule 1 (sICAM 1) and soluble vascular cellular adhesion molecules 1 (sVCAM 1) were measured in 52 patients with unstable angina (UA) during angina attack and relief period (24h after attack)as well as in 40 controls. Results The expression of leukocyte CD18 and concentrations of sICAM 1 and sVCAM 1 were significantly higher in UA patients than those in controls( P <0 001),the increase of the expression of leukocyte CD18 and concentrations of sICAM 1 and sVCAM 1 in angina attack were much obvious than those in relief period ( P <0 001). Leukocyte CD18 expression correlated positively with concentrations of sICAM 1 and sVCAM 1( r =0 568,0 624,0 669,0 746, P <0 001). In different types of angina, there were significant difference of leukocyte CD18 expression and concentrations of sSICAM 1 and sVCAM 1 in angina attack and relief periods ( P <0 001)and the increase of all the indicators in spontaneous angina patients were more obvious than those that in exertional and postinfarction angina patients. Conclusions Acute myocardial ischemia might be related to the changes of CAMs.
