不输库血体外循环心内直视手术40例报道

摘　　要：报道 ４０例体外循环心内直视手术不输库血的临床经验。认为术前血红蛋白≥１１５ ｇ／Ｌ、红细胞压积比≥０． ３４的心脏病患者，通过严格的血液保护措施可以在其施行体外循环心内直视手术时做到不输库血。成功的血液保护措施有；严格控制输血指征；体外循环采用血液中度稀释；血液自体回输；术中保持足够的麻醉深度及术中严密止血等。用综合措施开展血液保护，安全、可行、有效，既节省了费用又避免了输库血而可能感染的传染性疾病。The successful experience of 40 cases of open heart surgery by cardiopulmonary bypass (CPB) without homologous blood transfusion was reported. It was considered that the patients with hemoglobin more than 115 g/L and hematocrit (HCT) more than 0. 34 undergoing open heart surgery with CPB would have no homologous blood transfusion by scientific blood conservation. The methods for blood conservation included: strictly control the indications of blood transfusion; CPB with middle dilution and transfusion of the remaining machine blood; maintaining an appropriate depth of anesthesia and carefully stopping bleeding during operation. The comprehensive techniques of blood conservation were safe and effective in open heart surgery, which might not only save the bank blood but also avoid the possible epidemical diseases from homologous blood transfusion.
