胰液肠内引流式胰肾同期联合移植(附一例报告)

摘　　要：目的　观察胰肾联合移植治疗胰岛素依赖型糖尿病合并肾功能衰竭的效果。方法　为１ 例胰岛素依赖型糖尿病合并肾功能衰竭的患者施行胰肾同期联合移植， 移植胰的外分泌采用肠内引流。结果　移植胰术后第１ ｄ 恢复正常功能， 移植肾术后第３ ｄ 恢复正常功能， 术后１ 周停用胰岛素， 空腹及餐后血糖正常， 未发生急性排斥反应及肠内引流术式的相关并发症。结论　肠内引流式同期胰肾联合移植是治疗糖尿病合并肾功能衰竭的较好术式。Objective To observe the efficacy of simultaneous pancreas kidney transplant to treat insulin dependent diabetes mellitus complicated with end stage renal failure. Methods The patient received simultaneous pancreas kidney transplantation with enteric drainage of exocrine pancreatic secretions. After transplantation, immunosuppressants including ALG, MP and Cy were used for 4 days and then ALG, MMF and Pred for maintenance. Results Pancreas graft and kidney graft recovered normal function 1 day and 3 days after grafting respectively. The patient became insulin independent and has normal renal function. Up to now no acute rejection of both organs and complication of enteric drainage occurred. Conclusion Simultaneous pancreas kidney transplantation with enteric drainage is better techniques for treatment of diabetes mellitus complicated with end stage renal failure.
