健康体检人群体脂肪含量与血压水平的关系

摘　　要：目的探讨体脂肪含量(BF%)与血压水平之间的关系。方法采用横断面研究的方法对健康体检者904例测量身高、体重、血压,检测血糖、血脂及血尿酸,利用生物电阻抗(BIA)原理进行BF%测定。结果高血压组BF%高于正常组。按BF%四分位分组,结果显示男女两性的收缩压、舒张压、平均动脉压及高血压检出率均随BF%水平升高而增高的趋势。多因素Logistic回归分析显示性别、甘油三酯、血糖、BF%、年龄和尿酸是高血压的危险因素。男性BF%、BMI和WHR判定高血压的ROC曲线下面积分别为0.696(95%CI:0.651～0.740)、0.707(95%CI:0.664～0.751)和0.762(95%CI:0.722～0.802)。女性分别为0.780(95%CI:0.720～0.840)、0.797(95%CI:0.735～0.859)和0.844(95%CI:0.796～0.893)。结论 BF%是高血压的危险因素,BF%对高血压诊断价值与BMI相近,略低于WHR。Objective To determine the relationship between body fat percentage and blood pressure.Methods We performed a cross-sectional study of 904 health examination people.Height,weight,blood pressure,blood glucose,blood lipid and uric acid were conducted in the fasting state.Body fat percentage（BF%）was measured by bioelectrical impedance analysis（BIA）.Results BF% was higher in the hypertension group than that in normal group.The level of systolic blood pressure（SBP）,diastolic blood pressure（DBP）and mean arterial pressure（MAP）and incidence of hypertension were increased significant with the level of BF% increased in both men and women.Multiple logistic regression showed that risk factors for hypertension were gender,triglyceride,fasting glucose,BF%,age and uric acid.The areas under the receiver operating characteristic curves of BF%,BMI and WHR in predicting the incidence of hypertension were 0.696（95% CI：0.651-0.740）,0.707（95% CI：0.664-0.751）and 0.762（95% CI：0.722-0.802）in men and 0.780（95% CI：0.720-0.840）,0.797（95% CI：0.735-0.859）and 0.844（95% CI：0.796-0.893）in women.Conclusions BF% is a risk factor for hypertension,diagnostic value of BF% for hypertension is similar to BMI and sightly lower than WHR.
