摘 要:目的 探讨肾移植术后内源性促红细胞生成素(EPO) 的动态变化与康复的关系。 方法 分别测定血透析和肾移植病例32 例。将移植病人按术前是否接受EPO 治疗,分为EPO 治疗组和非治疗组。动态观察手术前后EPO变化过程。比较治疗和非治疗组的差异与血象变化。 结果 两组各观察点EPO 实测值差异无显著性。EPO 治疗组术后可见两次EPO 升高高峰,分别为32mIU/ml 和21mIU/ml。贫血的改善不受EPO 变化的影响,呈稳定上升趋势,第21 天两组血红蛋白平均达到90g/L,平均术后8 天肾功能恢复正常。肾功能延迟恢复病例,欠缺二次EPO 水平升高过程。 结论 肾移植术后监测EPO水平,有助于预测和判断移植肾功能的康复和发生移植术后红细胞增多症(PTE)的可能性。Objective To study the inherent erythropoietin (EPO) before and after renal transplantation and to investigate its correlation to allograft function. Methods In 55 cases of chronic renal failure underwent hemodialysis or renal transplantation,the EPO level has been followed up and its correlation with anemia and the allograft function was investigated. Results There was no significant differrence of EPO level between the hemodialysis and the renal transplantation group and the inherent EPO level has nothing to do with the correction of anemia.The allograft function usually recovered 8 days after transplantation and peripheral blood hemoglobin rose smoothly.In exogenous EPO treated patients,2 peaks of EPO level were observed after renal transplantation.If delayed recovery of the allograft function occurred,the second peak of EPO was dismissed. Conclusions Checking up of the inherent EPO level would be of help in estimating the allograft function after renal transplantation and in forecasting the possible occurrecnce of post transplantation erythrocytosis.
