中西医结合治疗甲型H1N1流感重症病例21例临床观察

摘　　要：目的:总结中西医结合治疗甲型H1N1流感重症患者的疗效,探讨甲型H1N1流感重症的临床特点和救治措施。方法:对21例甲型H1N1流感重症患者全部在西医常规治疗的基础上给予中医辨证施治。每日观察记录症状、体征、中医证侯评分、实验室检查、胸部或腹部影像学检查。结果:治愈12例(57.14%),好转9例(42.86%),总有效率100%,没有1例转为危重症。甲流病毒核酸转阴时间平均为5.44天,平均住院时间6.38天,平均退热时间22.76小时,平均症状缓解时间104.10小时。中医证型以"痰热壅肺或痰浊阻肺证"最多(9例),其次为"湿温证"(6例)、"凉燥证"(4例)、"温燥证"(1例)、"湿温夹秋燥证"(1例),多夹痰湿,治疗上注重化痰除湿。结论:四川地区甲型H1N1流感重症患者大多存在痰湿证侯,中医治疗关键在于宣肺化痰、健脾除湿,中西医结合疗法有良好的治疗效果。Objective： Summarizing the curative effect of Integrating Traditional Chinese Medicine and Western Medicine on Treating 21 cases of seriously Influenza A （ H1N1） , discussing the chnical characteristics and treatment measures of the seriously Influenza A （ H1N1 ）. Methods： Giving 21 cases of seriously Influenza A （H1N1） dialectical executes medicine on the basis of conventional treatment and daily observation of symptoms, signs, laboratory examinations, the chest or abdomen imaging. Re- suits： After treatment, 12 cases were healed, 9 cases were better, all patients better discharge, no one was into critical case. The viral nucleic acid of influenza A （ H1N1） turn to negative for an average of 5.44days, the average hospitalization time was 6. 38 days, the average antifebrile time was 22. 76 hours, symptoms relieved for an average of 104. 10 hours. TCM syndrome type of 21 patients were really ＂phlegm＂, mixed up with ＂Tanre Yongfei or Tanzhuo Zufei＂, ＂Shiwen＂, ＂Wenzao＂, ＂Liangzao＂ and so on, with ＂Erchen Decoction＂ basis to add and subtract. Conclusion： The seriously Influenza A （H1N1） mostly has clinical manifestation of ＂Tanshi＂, basic principle of TCM therapy is ＂Xuanfei Huatan and Jianpi Chushi＂. Integrating Traditional Chinese Medicine and Western Medicine on Treating seriously Influenza A （ H1N1 ） has good effect.
