术前口服非那雄胺对绿激光前列腺气化术气化效率的影响

摘　　要：目的探讨术前口服非那雄胺对绿激光前列腺气化术(PVP)气化效率的影响。方法中重度前列腺增生患者150例,随机分成三组:A组术前不予口服非那雄胺,直接行PVP术;B组术前口服非那雄胺2～4周,再行PVP术;C组术前口服非那雄胺6个月以上,再行PVP术。比较三组患者的手术时间、术中出血量和激光气化效率。结果 A、B、C三组患者术中出血量分别为(43.5±15.1)、(35.7±13.4)和(32.6±11.3)ml;手术时间分别为(108.7±31.2)、(90.5±30.3)和(98.2±29.8)min;激光气化效率分别为(7.4±0.8)、(6.9±0.6)和(7.2±0.7)KJ/cm3。B、C两组患者的术中出血量较A组明显减少(P<0.05);而B、C两组比较,术中出血量差异无显著意义(P>0.05)。与A组比较,B组患者的手术时间明显减少(P<0.05),激光气化效率显著提高(P<0.05);但A、C两组比较,以及B、C两组比较,手术时间和激光气化效率差异无显著意义(P>0.05)。结论术前口服非那雄胺2～4周或6个月以上均可有效减少PVP术中出血量,术前短时间(2～4周)口服非那雄胺可提高PVP术激光气化效率,缩短手术时间,但术前长期(6个月以上)口服非那雄胺对此则无明显帮助。Objective To study the effect of finasteride taken preoperatively on efficiency of photoselective vaporization of prostate（PVP）. Methods Altogether 150 patients of benign prostatic hyperplasia（BPH） with operation indication were randomly divided into three groups： group A,in which the patients were given PVP directly without taking finasteride preoperatively,group B,in which the patients were given PVP operation after they had taken finasteride for 2-4 weeks,and group C,in which the patients were given PVP operation after they had taken finasteride for 6 months at least.A comparative study on operation duration,blood loss during operation,and efficiency of vaporization was made for the three groups. Results In group A,B and C,the blood loss during operation were（43.5±15.1）ml,（35.7±13.4）ml and（32.6±11.3）ml respectively,the operation duration（108.7±31.2） min,（90.5±30.3） min and（98.2±29.8） min respectively,and the efficiency of vaporization（7.4±0.8） KJ/cm3,（6.9±0.6） KJ/cm3 and（7.2±0.7） KJ/cm3 respectively.Compared with group A,the blood loss during operation were much less in group B and C（P0.05）,but the difference between group B and C was not significant.The operation duration was much shorter and the efficiency of vaporization much higher in group B（P0.05） than in group A.No significant difference in operation duration or vaporization efficiency was observed either between group A and group C or between group B and group C. Conclusions Both short-term（2-4 weeks） and long-term（at least 6 months） finasteride administration before PVP can effectively decrease blood loss in PVP operation.Short-term（2-4 weeks） finasteride administration before PVP can improve vaporization efficiency and shorten operation duration.On the other hand,the effect of long-term（at least 6 months） finasteride administration before PVP is very limited in this respect.
