摘  要:目的建立兔放射性骨损伤模型,动态观察受照射骨髓的影像学变化,探讨CT能谱成像(gemstonespectralimaging,GSI)对放射性骨损伤的诊断价值。方法共用健康成年新西兰大白兔16只制作下肢放射性骨损伤模型,采用直线加速器X射线30Gy体外单侧单次照射兔后肢全长,于照射后1、2、4、6、8、12、18周行宝石能谱CT扫描。采用物质分离技术定量测定髓腔内脂质及水含量,定期行病理学检查。结果兔股骨及胫骨上段照射后1周髓腔内脂含量显著升高。2-4周逐步下降,4-8周趋于平稳,8-18周呈显著升高趋势。相应地,髓腔内水含量第1周显著下降,第2周明显升高,2-12周趋于平稳,稍有下降,12-18周后呈逐步下降趋势。能谱CT测量结果与病理学表现相吻合。结论奉研究成功建立了兔放射性骨损伤模型,CT能谱成像能够定量测定髓腔内脂水含量,可用于放射性骨损伤的早期监测及随访。Objective To establish a rabbit model of radioactive bone injury and to investigate the value of gemstone spectral ima- ging(GSI) in diagnosis of radioactive bone injury. Methods Sixteen healthy and mature New Zealand rabbits received irradiation on the unilateral hind limb with single fraction 20 gray X-rays to establish rabbit models of radioactive bone injury. The hind limbs were scanned at 1,2,4,6,8,12,and 18 weeks after the irradiation byCT. The marrow lipid and water content were measured quantitative ly using material separation technique, and then the pathological examinations of the limbs were done. Results Tbe lipid component of medullary cavity of the limb was significantly increased after exposure to radiation 1 week, then gradually declined during 2--4 weeks, stabilize 4-8 weeks, while significant increasing trend 8--18 weeks later. After exposure to radiation, the water component of marrow was decreased significantly within the first weeks, increased significantly at the second seek, stabilized during 2 to 12 week and gradually declined after 12- 18 week. The results measured with spectrum CT was consistent with that of patbology. Con- clusion The rabbit models of radiation bone injury are established successfully in this study. GSI can quantitative determine lipid and water components, it can be used to detect radioactive bone injury early and as follow up method.
