影响突发性耳聋临床疗效的相关性因素分析

摘　　要：目的:对住院治疗出院的突发性耳聋患者的临床病例进行回顾性研究,探讨各种相关因素对预后的影响,为临床疗效的预判提供科学依据。方法:对泸州医学院附属医院2006年2月至2008年2月收治的资料完整的120例突发性耳聋病例,进行了回顾性研究和统计学分析。治疗前和治疗后进行纯音听阈测试,以0.5kHz、1.0kHz、2.0kHz及4.0kHz的气导平均值(平均听阈PTA)作为评价的标准,临床疗效与患者的年龄、性别、治疗前的听力水平、是否伴有耳鸣等因素进行相关性分析。经SPSS12.0进行χ2检验、t检验和U检验,P<0.05时则认为具有统计学意义。结果:患者的年龄、性别、患者是否伴有耳鸣、患者发病的月份均与疗效无关(P>0.05),而是否伴有眩晕、发病到治疗的时间和发病前的PTA水平与疗效的关系较为密切(P<0.05)。结论:早期治疗突发性耳聋是提高疗效的关键,病程长短可为预后预测提供参考。突发性耳聋不伴有眩晕的患者疗效较好。Objective： To analyze the factors influencing therapeutic effects of sudden deafness and their relation with prognosis through retrospective analysis of clinical cases. Methods： The data of one hundred and twenty patients diagnosed as sudden deafness from 2006 to 2008 was analyzed. The patients were tested by pure tone audiometer before and after the therapy. The effect of therapy was assessed by the average of air-conductive hearing level at 0.SkHz,l.0kHz,2.0kHz and 4.0kHz,the correlation between therapeutic effect and influencing factors （such as sex, age, hearing level of patients before treatment,promptness of treatment,and whether with or without tinnitus）was analyzed. Results： The therapeutic effects of sudden deafness had no relation with sex, age, and tinnitus（P〉0.05）, but related with hearing level of patients before treatment, the promptness of treatment, and whether with or without vertigo （P〈0.05）. Conclusion： Getting treatment as early as possible is the key point to improve the therapeutic effect. In addition, the patients with sudden deafness and without vertigo have good therapeutic effect.
