摘  要:目的分析颧骨复合体骨折不同固定方式下功能负载时颧骨体上的应力分布情况,结合临床对各种固定方式进行评价。方法在颧骨复合体骨折固定三维有限元模型上,加载模拟咀嚼肌及表情肌牵拉力,应用ANSYS软件分析7种不同固定方式下颧骨体上的应力分布情况。结果单点固定时颧骨复合体上的应力主要集中在固定钛板周围,两点、三点固定时颧骨复合体上的应力进一步分散。结论采用单点、两点或三点固定颧骨复合体都有获得稳定固定效果的可能。采用何种固定方式常结合颧骨体复位的手术入路决定,以最小手术创伤获得稳定固位效果为原则。Objective To evaluate the types of fixation for zygomatic complex fracture by three dimensional finite element stress analysis and discuss its clinical feasibility. Methods Three dimensional finite element model of zygomatic complex fracture was constructed and the stress distributions on the repositioned zygomatic complex which frxed with seven types of combinations simulating mini-plate fixation at front zygomtttic, infraorbital and zygomaticomaxillary buttress were analyzed. Results The stress concentrates on the fixation site if the zygomatic complex was fixed with single mini-plate. In contrary, the stress on the zygomatic complex dispersed gradually after fixation with two or three mini-plates. Conclusions It is possible to get stable conditions with all seven types of fixation on the zygomatic complex fractures. It is the principal that stable fixation can be established by the minimally surgery intervention for management of zygomatic complex fractures.
