摘 要:目的探讨慢性阻塞性肺疾病(COPD)稳定期患者和肺功能正常吸烟者外周血中Th17细胞的表达及意义。方法选取不吸烟肺功能正常者(A组)、吸烟肺功能正常者(B组)、轻中度(C组)及重度(D组)COPD稳定期患者为研究对象。流式细胞术检测外周血单个核细胞(PBMC)中Th17细胞的百分比,ELISA检测血浆及PBMC刺激培养液中白介素(IL)-17的水平。结果 B、C和D组外周血Th17细胞、IL-17及均PBMC刺激培养液中IL-17水平高于A组(P<0.01);C组和D组外周血Th17细胞和IL-17均高于B组(P<0.01);D组外周血及PBMC刺激培养液中IL-17水平均高于C组(P<0.01);D组PBMC刺激培养液中IL-17水平高于B组(P<0.01)。C、D组血浆及PBMC刺激培养液中IL-17水平与第一秒钟用力呼气容积(FEV1)/预计值之间呈负相关;各研究组外周血和PBMC刺激培养液中IL-17水平均与外周血Th17细胞水平呈正相关;所有吸烟者外周血Th17细胞水平与吸烟年数呈正相关,C、D组外周血Th17细胞水平与吸烟年数无相关性。结论 Th17细胞及IL-17在COPD稳定期患者和肺功能正常吸烟者外周血中表达增加。肺功能正常吸烟者外周血Th17细胞水平与吸烟年数相关;COPD稳定期患者外周血IL-17与FEV1/预计值呈负相关,Th17细胞与FEV1/预计值及吸烟年数无相关性。Objective To study the expression and significance of peripheral blood Th17 cells in patients with sta- ble chronic obstructive pulmonary disease(COPD) and normal lung function smokers. Methods Choose non-smok- ers with normal lung function ( A group, 18 cases), smokers with normal lung function ( B group, 20 cases) , sta- ble mild to moderate (C group, 19 cases) and severe (D group, 15 cases) COPD patients in our study. Percent- age of Thl 7 cells in peripheral blood mononuclear cells (PBMC) was determined by flow eytometry. IL-I 7 levels in plasma and PBMC supernatant after stimulation were measured by ELISA. Results The percentage of Th17 cells, the levels of IL-17 in peripheral blood and PBMC supernatant after stimulation of group B, group C and group D were higher than those of group A(P 〈 0. 01 ). The percentage of Th17 cells and levels of IL-17 in peripheral blood in group C and D were higher than those of group B(P 〈0. 01 ). The levels of IL-17 in plasma and PBMC superna- rant after stimulation in group D were increased significantly compared with those of group C ( P 〈 0. 01 ). The level of IL-17 in PBMC supernatant after stimulation in group D was higher than that in group B(P 〈0.01 ). In group C and D, the levels of IL-17 in plasma and PBMC supernatant after stimulation were negatively correlated with FEV1 (% of predicted value). The levels of IL-17 in plasma and PBMC supernatant after stimulation were positively cor- related with the percentage of Th17 in PBMC. The percentage of Th17 in PBMC was positively correlated with the years of smoking in all smokers. The percentage of Th17 in PBMC had no relation with the years of smoking in group C and group D. Conclusion The levels of Th17 cells and IL-17 in peripheral blood increased in patients with stable COPD and smokers with normal lung function.
