摘 要:目的:通过持续质量改进(CQI)提高住院药房调剂质量,提升药房管理水平。方法:采用回顾性分析和前后对照的研究方法,找出我院住院药房调剂过程中各环节存在的问题和缺陷,实施相应CQI措施(如实行目视管理、完善医院信息系统程序、合理排班等),并对比实施前(2010年5-8月)、后(2010年9-12月)的各类差错(如"漏药品"、"数量错"等)数据,评价CQI的效果。结果:实施一系列CQI措施后,改进前差错例数居前列的"漏药品"和"数量错"分别从186、133例下降至92、65例,下降幅度均超过50%;口服药、注射剂2种摆药方式发生的差错例数均较改进前显著下降(从284、131例下降至127、49例,P<0.01),具有统计学意义。结论:长效的维护机制、不断的持续改进,是提高医院住院药房调剂质量的重要保障,是提升药房管理水平的基石。OBJECTIVE:To improve the quality of dispensing works in the inpatient pharmacy by applying continuous quality improvement(CQI) and to promote pharmacy management.METHODS:Retrospective analysis and control study were used to discover various issues related to dispensing errors in our hospital,and appropriate measures for continuous quality improvement were implemented(i.e.carrying out visual management,perfecting HIS program,rational arranging).By comparing the data of the before(May-Aug.2010) and after(Sept.-Dec.2010) implementation,the effects of continuous quality improvement were evaluated.RESULTS:After applying CQI,the number of dispensing errors caused by leakage and wrong number of drugs were decreased from 186 cases and 133 cases before implementation to 92 cases and 65 cases after implementation,decreasing more than 50%;the number of dispensing errors of oral preparation and injection caused by 2 types of dispensing ways were both significantly decreased(decreasing from 284 cases and 131 cases to 127 cases and 49 cases,P〈0.01),there was statistical significance.CONCLUSION:Long-term maintenance and CQI guarantee the quality of dispensing work and lay a foundation of enhancing the management of inpatient pharmacy.
