摘 要:目的探讨血清中血管内皮细胞生长因子(vascular endothelial growth factor,VEGF)和C反应蛋白(CRP)与糖尿病心肌病(diabetic cardiomyopathy,DC)发病的关系。方法选择广州军区武汉总医院内分泌科和华中科技大学心血管病研究所2007年1月至2010年1月收治的73例2型糖尿病(T2DM)患者和101例DC患者为研究对象,以40例健康体检者为正常对照组。采用酶联免疫吸附法(ELISA)测定3组受试者血清中VEGF和CRP水平。通过超声心动图评价DC患者心脏结构和心功能,分析心脏不同扩大程度、不同心功能与血清中VEGF和CRP水平的关系。结果 DC组血清中VEGF水平([312.2±12.2)ng/ml]明显高于T2DM组([98.7±4.6)ng/ml],差异有统计学意义(P<0.05)。T2DM组血清中VEGF水平明显高于正常对照组([40.7±2.2)ng/ml],差异有统计学意义(P<0.01)。DC组CRP水平([13.0±2.7)mg/L]高于T2DM组([7.0±0.9)mg/L]和正常对照组([4.3±0.2)mg/L],差异有统计学意义(P<0.05,P<0.01)。DC患者心脏中度扩大和重度扩大组VEGF和CRP水平均高于轻度扩大组,差异有统计学意义(P<0.05,P<0.01)。DC患者心脏收缩功能不全+舒张功能不全组VEGF和CRP水平[分别为(401.1±6.32)ng/ml、(17.7±1.47)mg/L]高于单纯心脏舒张功能不全组[分别为(236.1±17.9)ng/ml、(11.1±0.7)mg/L],差异有统计学意义(P<0.05)。结论 DC发病可能与VEGF和CRP的参与有关,VEGF和CRP在DC发生发展中起着重要作用。Objective To investigate the relationship of the occurrence and development of diabetic cardiomyopathy (DC) with serum vascular endothelial growth factor(VEGF) and C reactive protein(CRP) in patients with type 2 diabetes mellitus (T2DM). Methods The levels of serum VEGF and CRP were measured by ELISA in 101 patients with DC, 73 cases with T2DM and 40 cases of the normal control. With double-blind method, echocardiography was used to compare left ventricular extent in all patients with DC. Results The levels of serum VEGF and CRP in patients with DC were significantly higher than those in patients with T2DM (P〈0.01) and in the normal controls (P〈0.05). The levels of serum VEGF and CRP in DC patients with middle and severe degree of heart expansion were higher than those in DC patients with mild degree of heart expansion(P〈O.05, P〈0.01 ). And the levels of serum VEGF and CRP were significantly higher in DC patients with left ventricular systolic dysfunction complicated with left ventricular diastolic dysfunction than those with the simple left ventricular diastolic dysfunction (P〈0.01). Conclusion There are higher elevated serum VEGF and CRP in patients with diabetic myocardial disease. The elevated serum VEGF and CRP may nlav imnortant roles in the natho^enesis of diabetic eardiomvooathv.
