摘  要:目的:探讨母牛分枝杆菌菌苗对糖尿病并肺结核患者免疫功能低下及其治疗效果的影响。方法:以糖尿病并肺结核患者为研究对象,CD4+淋巴细胞低下者随机分为治疗组和对照组Ⅰ,CD4+淋巴细胞正常者为对照组Ⅱ,3组均予以降糖和抗痨治疗,治疗组联用母牛分枝杆菌菌苗,比较分析3组患者治疗2个月后痰菌阴转,肺部病灶吸收情况。结果:在强化治疗1个月末和2个月末,对照组Ⅱ痰菌阴转率均较高(59.0%、75.0%)。空洞缩小闭合率以治疗组最高(92.0%)。治疗有效率以治疗组最高(72.4%)。结论:免疫功能低下的糖尿病并肺结核患者联用母牛分枝杆菌菌苗治疗,可促进痰菌阴转和空洞愈合。Objective To study the effects of vaccae-assisted therapy on the immune status of patients with diabetes mellitus complicated with pulmonary tuberculosis. Methods Patients with diabetes mellitus complicated with pulmonary tuberculosis were included in this study, patients with low level of CD4~ lymphocytes were randomly divided into treatment group and control groups I , and those patients with normal level of CD4+ lymphocytes were included as control group II. Patients in all the 3 groups received hypoglycemic and antituberculosis treatments, those in treatment group received Mycobacterium vaccae additionally. Sputum negative transfer and absorption of pulmonary lesions in the 3 groups were compared after the treatment. Results Sputum negative transfer rate at the end of 1 month and 2 month treatments (59.0% and 75.0%, respectively) were higher than that in control group II . The rates of cavity close and effective rates in treatment group were the highest (92.0% and 72.4% , respectively). Conclusion Vaccae-assisted treatment could promote the cavity close rate and sputum negative transfer rate in diabetic patients with cellular immune hypofunction complicated with pulmonary tuberculosis.
