摘 要:目的探讨利多卡因预先给药对大鼠’肾脏缺血再灌注时肾组织高迁移率族蛋白-1(ITMGB1)表达的影响。方法健康成年雄性Wistar大鼠36只,体重300—350g,采用随机数字表法,将其随机分为3组(n=12):假手术组(s组)、肾脏缺血再灌注组(I/R组)和利多卡因组(L组)。I/R组和L组采用夹闭双侧。肾动脉60min恢复灌注的方法建立肾脏缺血再灌注损伤模型。L组于缺血前60min静脉注射利多卡因5mg/kg,随后以2mg·kg^-1·h^-1速率静脉输注60min;I/R组给予等容量生理盐水。3组分别于再灌注4、24h时取6只大鼠,取肾组织,采用PCR技术检测HMGB1mRNA表达,采用Westernblot法测定HMGB1表达,分别采用黄嘌呤氧化酶法和硫代巴比妥法测定超氧化物歧化酶(SOD)活性和丙二醛(MDA)含量,光镜下观察肾组织病理学结果。结果与S组比较,I/R组和L组肾组织HMGB1mRNA及其蛋白表达和MDA含量升高,。肾组织SOD活性降低(P〈0.05);与I/R组比较,L组肾组织HMGB1mRNA及其蛋白表达和MDA含量降低,肾组织SOD活性升高(P〈0.05)。L组肾组织病理学损伤轻于I/R组。结论利多卡因可减轻大鼠肾脏缺血再灌注损伤,其机制与下调肾组织HMGB1表达有关。Objective To investigate the effect of lidocaine pretreatment on renal HMGB1 exPression dur- ing renal ischemia-reperfusion (I/R) in rats.Methods Thirty-six male Wistar rats weighing 300-350 g were randomly divided into 3 groups ( n = 12 each) : sham operation group (group S), I/R group and lidocaine pretreatment group (group L). Renal I/R was induced by occlusion of bilateral renal arteries for 60 rain followed by 4 or 24 h reperfusion. Lidocaine 5 mg/kg was injected iv at 60 min prior to ischemia followed by 2 rag" kg- 1. h- 1 infu- sion iv for 60 rain in group L. Equal volume of normal saline was given in group I/R. Six rats in each group were sacrificed at 4 or 24 h of reperfusion 'and their kidneys were removed for microscopic examination and for determi- nation of SOD activity, MDA content and the expression of HMGB1 mRNA and protein. Results Compared with group S, renal HMGB1 mRNA and protein expression, MDA content were significantly increased, while SOD activity were significantly decreased in groups I/R and L( P 〈 0.05). Compared with group I/R, renal HMGB1 mR- NA and protein expression, and MDA content were significantly decreased, while SOD activity were significantly increased in group L ( P 〈 0.05). Conclusion Lidocaine pretreatment can attenuate renal I/R injury in rats by down-regulating HMGB1 expression
