主动脉瓣二叶畸形患者左心室心肌工作指数的临床研究

摘　　要：目的:观察不同年龄组主动脉瓣二叶畸形(BAVD)并狭窄患者左心室心肌工作指数(MPI)的变化。方法:将40例超声心动图诊断为主动脉瓣二叶畸形并狭窄的患者分为2个年龄组,20～40岁组20例(轻度狭窄16例,中度狭窄4例),40岁以上组20例(轻度狭窄11例,中度狭窄7例,重度狭窄2例),并设立正常对照组各20例。频谱多普勒测量Ra值及Rb值,计算不同年龄组主动脉瓣二叶畸形患者左心室心肌工作指数,并与正常组对照。结果:20～40岁组心肌工作指数为(0.43±0.12),对照组为(0.32±0.09)r,=0.86(P<0.01);40岁以上组心肌工作指数为(0.54±0.13),对照组为(0.36±0.11)r,=0.91(P<0.01);2组比较r=0.85(P<0.01)。结论:随着年龄的增长,BAVD并狭窄患者的心功能有恶化倾向。Objective： To observe myocardial performance indexes （MPI） of left ventricle in patients with bicuspid aortic valve deformity （BAVD） and stenosis in different ages. Method： Forty patients proved with bicuspid aortic valve deformity and stenosis were divided into 20 cases in group of 20 to 40 years（16 cases of mild stenosis, four cases of moderate stenosis） and another 20 cases in group of above 40 years old （11 cases of mild stenosis, seven cases of moderate stenosis and two cases of severe stenosis）, 20 cases in normal control group were also set. Ra and Rb were measured by Doppler, MPI and BAVD in different groups were calculated and compared with control group. Result： MPI of 20-40 years group was（0.43±0.12）, MPI of control group was（0.32±0.09）, r=0.86 （P〈0.01）; MPI of above 40 years group was（0.54±0.13）, control group was（0.36±0.11）, r=0.91 （P〈0.01）; comparison of both groups showed r=0.85 （P〈0.01）. Conclusion： Heart functions in patients with BAVD and stenosis can deteriorate with the growth of age.
