肺功能检查在单纯型支气管内膜结核诊断中的作用

摘　　要：目的探讨单纯型支气管内膜结核(EBTB)患者肺功能的变化特点,提高早期诊断率。方法分析我科2008年—2011年间收治的20例单纯型EBTB患者的肺功能改变特点,选取同期健康体检者20例作为正常对照组进行比较。结果①EBTB组患者第1秒用力呼气量、第1秒用力呼气量与用力肺活量比值较对照组明显降低(P<0.05);但2组肺活量、用力肺活量比较无显著性差异(均P>0.05);②EBTB组患者约60%存在通气功能障碍,表现为阻塞性通气功能障碍、限制性通气功能障碍和混合性通气功能障碍;③25%阻塞性或混合性通气功能障碍患者支气管舒张试验阳性;典型的EBTB患者,其流速-容量曲线形态具有一定的特征性。结论大多数单纯性EBTB患者存在肺功能改变,以阻塞性通气功能障碍为主,且少数支气管舒张试验可阳性,流速-容量曲线形态常呈"双凹"形。Objective To investigate the change of lung function in patients with simple endobronchial tuberculosis （EBTB）and improve the diagnosis rate. Methods The lung function tests were performed in 20 patients with simple EBTB （ EBTB group）from 2008 to 2011 and 20 healthy people as experimenters （control gronp）.Results FEVl ,FEV1/FVC in EBTB group were significantly lower than those in control group （P〈0.05）.While there was no statistical differences of VC and FVC between two groups （/〉〉0.05）.15 cases in EBTB group （60%） showed abnormal ventilator,/ function comprehended：obstructive ventilatory function disturbance,ventilatory function disturbance and mixed ventilatory function disturbance. 4 cases （25%）with obstructive or mixed ventilatory function disturbance who received brochodilation test were positive.Some patients in EBTB group who had abnormal vontilatory function had characteristic changes in flow-volume curve. Conclusion Most patients with simple EBTB have abnormal ventilatory function which include mainly obstructive ventilator,/function disturbance. Brochodilation test may be positive and sometimes flow-volume curve has characteristic changes which show “double-concave”.
