摘  要:目的观察大鼠脑外伤后脑组织与外周血中血栓调节蛋白(TM)、血管假性血友病因子(vWF)的表达、脑含水量以及微血管面密度(MVD)的动态变化,探讨TM、vWF与MVD及脑含水量的相关性。方法 90只Wistar大鼠随机分为对照组和外伤组,采用自由落体打击法制做大鼠脑外伤模型。按伤后处理时间各组再分为9个亚组,每组5只,分别于30 min、1 h、3h、6 h、12 h、1 d、3 d、7 d、14 d不同时间点处死大鼠。采用干湿重法检测损伤侧脑组织含水量;免疫组化法测定脑组织中TM、vWF表达,CD34标记损伤灶周围脑皮层区微血管并计算MVD,酶联免疫吸附(ELISA)法检测外周血中TM、vWF的表达。结果大鼠颅脑创伤后外伤组脑组织及外周血中TM、vWF表达水平与对照组比较有明显升高(P<0.05),脑组织TM、vWF表达均与MVD呈负相关(r=-0.378,P<0.05、r=-0.426,P<0.05);血浆TM、vWF表达与MVD呈负相关(r=-0.663,P<0.05、r=-0.884,P<0.05)。结论 TM、vWF的表达水平变化可以反映脑外伤早期微血管内皮细胞活化状态,并且与脑水肿的发生发展密切相关。Objective To observe the expression of thrombomodulin(TM) ,von Willebrand factor (vWF) in brain and peripheral blood. And also to the change of study MVD and brain edema. In order to evaluate the correlation expression of TM, vWF with brain edema, and MVD. Methods 90 Wistar rats were randomly divided into sham operated and traumatic group. Refering to the free falling body crash device of Feeney, the head injury model was established. Every sub - group was composed of 5 rats and killed at the point of 30min,1 h,6 h,12 h, 24 h,3 d,7 d,14 d after injury. Brain water content was measured by dry-wet weight method. TM and vWF expressiom in brain was measured by immunohistochemical techniques. MVD was counted by using CD34 marked vascular endothelial cell,The plasma levels of TM and vWF were examined by using of ELISA. Results Traumatic brain injury in rats after trauma brain tissue and peripheral blood of TM and vWF expression levels were significantly higher ( P 〈 0. 05 ) , and MVD there was a negative correlation ( r = - 0. 378, P 〈 0. 05 ,r = - 0. 426, P 〈 0. 05 ) in brain and ( r = - 0. 663, P 〈 0. 05, r = - 0. 884, P 〈 0. 05 ) in peripheral blood. Conclusion The activation of microvascular endothelial cells of early traumatic brain injury can affect the microcirculation of blood to increase the development of cerebral edema.
