血管内超声iSize功能指导下行冠状动脉介入治疗的效果

摘　　要：目的:探讨血管内超声(IVUS)iSize功能诊断冠状动脉左主干及前降支根部病变的效果,优势及预后情况。方法:入选冠状动脉左主干及前降支根部有病变且在IVUS指导下植入支架的患者(A组,30例),及未行IVUS检查,仅根据冠脉造影评估行支架植入术者(B组,30例)。比较两组患者支架植入情况及术后6个月的随访结果。结果:两组患者住院期间心肌梗死率和急诊冠脉旁路移植术率均无显著性差异(P>0.05);与B组比较,A组超敏C反应蛋白[(31.59±56.93)mg/L比(5.03±7.24)mg/L]、病死率(3.3%比0)、心绞痛复发(26.7%比6.7%)及再狭窄率(16.7%比3.3%)明显降低(P<0.05)。结论:应用IVUS指导左主干及前降支根部病变介入治疗,有助于选择更合适的治疗方案,减少组织损伤以及提高疗效,降低冠心病死亡率和冠脉再狭窄率。Objective： To study diagnotic effect on lesions of left main coronary artery （LM） and root of left anterior descending artery （LAD） by iSize function of intravascular ultrasound （IVUS）, its advantages and prediction for prognosis. Methods： A total of 30 patients with lesions in LM and root of LAD undergoing percutaneous coronary intervention （PCI） under isize functiolal guidance of IVUS were selected as group A. Another 30 patients, who didn＇t receive IVUS but only received evaluation of coronary angiography for PCI, were enrolled as group B. PCI results were compared between two groups. Results： There were no significant difference in incidence rate of myocardial infarction and rate of emergency coronary artery bypass graft during admission between two groups （P〉0.05） ; compared with group B, there were significant decrease in level of high sensitive C reactive protein [ （31.59 ± 56. 93） mg/L vs. （5.03 ± 7.24） mg/L], mortality rate （3.3% vs. 0）, recurrence rate of angina pectoris （26.7% vs. 6. 7%） and restenosis rate （16. 7% vs. 3.3%） in group A, P〈0. 05 all. Conclusion： Evaluation of lesions in LM and root of LAD by IVUS for PCI helps to choose more proper therapeutic plan, reduce tissue damage and improve instant and long term effects of stent implantation, decrease mortality rate and restenosis rate of coro- nary artery.
