喹那普利联合比索洛尔治疗扩张型心肌病的疗效分析

摘　　要：目的:观察喹那普利联合比索洛尔治疗扩张型心肌病(DCM)的临床疗效。方法:将76例DCM患者随机分为对照组及治疗组,每组38例,对照组予常规治疗,治疗组在常规治疗基础上加用喹那普利与比索洛尔,疗程为1年,观察2组治疗前后临床疗效、病死率、心功能、心脏结构的变化。结果:治疗组的总有效率为92.1%,高于对照组有效率63.2%(P<0.01);治疗组病死率7.9%,低于对照组28.9%(P<0.05);治疗组射血分数、每搏输出量、心率亦较对照组显著改善(P<0.01),治疗组左心室收缩末期内径低于对照组(P<0.01)。结论:喹那普利联合比索洛尔可显著提高DCM的临床疗效,改善心功能,降低死亡率,改善心脏结构,可在DCM治疗中推广应用。Objective ：To observe the efficacy of quinapril combined with bisoprolol in patients with dilated cardiomyopathy（DCM）. Methods：Seventy-six cases with DCM were randomly divided into control group and treated group ,38 cases in each group. The patients in control group were treated with conventional treatment, and others in treated group were treated with quinapril and bisoprolol on the basis of conventional treatment, and the course of treatment was for one year. The clinical efficacy, mortality, cardiac function and cardiac structural changes were observed before and after treatment. Results：After treatment, the total effective rate was 92.1% and mortality rate was 7.9% in treated group, which were significantly better than in control group, the total effective rate was 63.2% （P 〈 0.01 ） and mortality rate was 28.9% （ P 〈 0.05 ）. After treatment, the parameters of heart function were significantly improved than pre- treatment in treated group. Left ventriular endsystolic dimension were significantly reduced compared with control group （ P 〈 0.01 ）, stroke volume, heart rate and ejection fraction were improved significantly compared with control group（P 〈 0. 01 ）. Conclusions： .I Quinapril combined with bisoprolol can significantly lrnprove clinical efficacy of DCM and cardiac function, reduce mortality and improve cardiac structure, which can be further applied in treatment of DCM.
