高频彩色多普勒超声在乳腺良恶性肿块鉴别诊断中的价值

摘　　要：目的探讨高频彩色多普勒超声对乳腺良恶性肿瘤的鉴别诊断价值。方法采用高频二维超声、彩色多普勒超声成像技术对284例患者的362个乳腺肿块进行超声检查,分析病变的形态、边缘、内部回声、纵/横(L/T)以及有无钙化灶、血流信号分布等超声征象,并与术后病理结果对比。结果 98个恶性肿块中87个超声诊断为乳腺癌的与病理结果相符,有11个误诊为良性肿块,诊断符合率为88.8%;另264个良性肿块中239个超声诊断为良性肿块的与病理结果相符,有25个误诊为恶性肿块,诊断符合率为90.5%。结论高频二维超声结合彩色多普勒血流成像(CDFI)检查在乳腺良恶性肿瘤鉴别诊断中有重要的临床应用价值,联合有统计学意义的超声征象可明显提高对乳腺肿瘤良恶性的诊断准确率。Objective To evaluate the value of high frequency color Doppler ultrasound in the differential diagnosis of breast neoplasm. Methods Totally 362 nodes in 284 patients were evaluated with high frequency color Doppler ultrasound. The shape,edge, inner hoes, axial/transverse（L/T）ratio, calcification and Doppler signals of penetrating vessels in the breast masses were analyzed. The results were compared with those of pathology. Results The coincidence rates of high frequency color Doppler ultrasound to breast malignant node were 88.8 % （87/98）. The rates of high frequency color Doppler ultrasound to breast benign node were 90. 5% （239/264）Conclusion High frequency color Doppler ultrasound plays an important role in the differential diagnosis of breast neoplasm. The combination of statistical significance ultrasonographic signs can improve the differential diagnosis accuracy of benign and malignant breast neoplasm.
